Objective: To assess the cognitive function of patients with Atrial fibrillation(AF)and to reveal the protective factors and risk factors of Mild cognitive impairment(MCI)in atrial fibrillation(AF).Methods: 300 patients with atrial fibrillation in the Department of cardiovascular medicine and geriatrics department of the First Affiliated Hospital of Shihezi University from November 2016 to December 2017 were randomly selected to complete the clinical data collection and cognitive function evaluation.The main clinical data included: patient general condition,laboratory examination results,drug use,atrial fibrillation type(paroxysmal atrial fibrillation,persistent atrial fibrillation,permanent atrial fibrillation),combined with other diseases.CHA2DS2 VASc was used to assess the risk of thrombus.The daily life assessment scale(ADL scale)was used to evaluate the patient's daily life function,and the overall cognitive function of the patients was evaluated by the MOCA scale,including the cognitive conditions in different fields,such as visual space and execution,naming,attention,language,abstract,delayed memory,orientation force and so on.According to the MOCA score and the total score of the ADL scale,the group of atrial fibrillation combined with mild cognitive impairment(group MCI): ADL < 26,15 points < MOCA score < 24,and atrial fibrillation and normal cognitive function(control group): ADL < 26,MOCA score > 24.A total of263 cases entered the formal study.SPSS20.0 was used for statistical analysis.P < 0.05 was statistically significant.Results:1.Inclusion group: the study included 263 subjects,159 males(60.45%)and 104 females(39.54%),with an average age of 65.83 + 11.54 years.Paroxysmal atrial fibrillation occurred in 79 patients(30.04%),persistent atrial fibrillation in 71 patients(27%),and permanent atrial fibrillation in 123 patients(46.77%).The top three cases were 122 cases of hypertension,46.38% cases of coronary heart disease,108 cases of coronary heart disease(41.06%),60 cases of diabetes mellitus(22.81%).The average score of total Mo CA score in 263 patients with atrial fibrillation was(20.91 + 3.61),lower than the critical value of 24.The Mo CA score in group MCI was lower than that of the control group,with statistical significance.Compared with the group,there were significant differences in visual space and execution,naming,attention,language,abstract,delayed recollection and orientation force(P < 0)..05).There were 189 cases in group MCI,including 113 male and 76 female,with an average age of 67.42 + 10.83 years,and 74 cases in the control group,including 46 men and 28 women,with an average age of 61.01 + 12.59 years.2.Comparison of clinical data: two groups found that atrial fibrillation type,CHA2DS2 VASc score,coronary heart disease,diabetes,COPD,dabiga group,statins,radiofrequency ablation history,FIB,D-two polymer,Hcy,left atrial diameter and head MRI/CT were statistically different among the two groups(P <0.05).The correlation analysis of MCI related factors and MOCA score showed that the Mo CA score was negatively correlated with age,CHA2DS2 VASc score,FIB,D-two polymer,Hcy,left atrial diameter,head MRI/CT,coronary heart disease,diabetes and COPD,and had a positive correlation with the history of higher education,dabiga group and radio frequency ablation(P <0.05).Multifactor analysis of significant related factors in single factor analysis showed that high years of education and dabiga group were protective factors for atrial fibrillation combined with MCI,while age,FIB,D-two polymer,left atrial diameter and diabetes were risk factors for atrial fibrillation combined with MCI.The probability of occurrence of MCI in paroxysmal tremor,persistent atrial fibrillation and permanent atrial fibrillation gradually increases.Conclusion:1.Age,CHA2DS2 VASc score,FIB,D-two polymer,Hcy,left atrial diameter,head MRI/CT,coronary heart disease,diabetes,COPD were negatively correlated with Mo CA scores.Higher education,the use of dabigatran,history of radiofrequency ablation were positively correlated with Mo CA score.2.The use of dabigatran and high education years were protective factors for AF and MCI;age,FIB,D-dimer,left atrial diameter,and diabetes were risk factors for AF and MCI.
