| Objectives:Analyze the risk factors of carotid atherosclerosis and investigate the relationship between 25-hydroxyvitamin D3 and carotid atherosclerosis in patients with type 2 dia-betes mellitus by analyzing the concentrations of 25-hydroxyvitamin D3.Methods:In this study,238 clinically diagnosed Type 2 diabetes mellitus patients were divi-ded into two groups which is type 2 diabetes mellitus without carotid atherossclerosis group(Non-CAS group,94 cases)and type 2 diabetes mellitus complicated with caroti-d atherosclerosis group(CAS group,144 cases)according to IMT value of the carotid artery.The general information of patients was collected,including gender、age、dur-ation of diabetes、height、weight、blood pressure and the history of smoking、 hype-rtension and coronary heart disease.Fasting venous blood samples were taken from all patients to estimate their FBG、TC、TG、HDL-C、LDL-C、LPA2、Ca、Insulin、OCN and 25(OH)D3 by Automatic biochemical analyzer and automatic chemilumine-scence instrument.Statistical analyses were performed using SPSS19.0 Statistics.The variables conforming to normal distribution are expressed in`x±s deviotion and com-parised by t test;conunting dates using chi square test.Analyze the risk factors of caro-tid atherosclerosis and investigate the relationship between 25-hydroxyvitamin D3 and carotid atherosclerosis in patients with type 2 diabetes mellitus by Logistic regression analysis.Results:(1)The t-test indicates that the age,the duration of diabetes,BMI and the concen-tration of LDL-C、LAP2、Hb A1c、HOMA-IR and OCN in CAS group were sign-ificantly different from the Non-CAS group(P<0.05),other indices such as SBP、DBP、TC、TG、HDL-C、Ca and FBG levels showed no significant difference(P>0.05).The Chi square test showed the history of smooking,hypertension and coronary heart disease have significantly different between two groups(P<0.05).(2)The 25(OH)D3 status were significantly lower in CAS group compared to Non-CAS by t test(P<0.05).All patients are divided into groups of vitamin D deficiency and sufficiency with of 25(OH)D3 equal to 50nmol/L as the cutoff value,results showed that 25(OH)D3 deficiency patients at risk for cervical atherosclerosis is2.899 times that 25(OH)D3 deficiency patients.(3)The logistic regression analysis showed that the relationship between25(OH)D3 and carotid atherosclerosis in type 2 diabetic after adjusting the influence of gender、age and other factors.The hierarchical multiple regression indicated thatwith severe 25(OH)D3 deficiency were 4.754 times increase odds of being T2 DM with CAS while those with mild to moderate deficiency were 1.535 times increase odds of being T2 DM with CAS.Conclusion:(1)Vitamin D deficiency is prevalent in Type 2 diabetes mellitus,the CAS group patients serum 25(OH)D3 levels was less than that the Non-CAS groups.(2)With the increase of age、during of disease、HOMA-IR 、LPA2and decreased of OCN level,the risk factors of carotid atherosclerosis are increased in type 2 diabetic patients.(3)The level of serum 25(OH)D3 in patients with T2 DM was negatively correlat-ed with the levels of TC,LDL and OCN.(4)There is association between 25(OH)D3 and carotid atherosclerosis in patients with type 2 diabetes mellitus.The level of serum 25(OH)D3 decreased,the risk of caro-tid atherosclerosis in type 2 diabetic patients increased. |
