Surgery Timing Of Transanal Endorectal Single-stage Definitive Operation For Neonataes With Hirschsprung's Disease

Objective:To study the surgical outcomes of children with congenital Hirschsprung's disease treated with one-stage anal resection of the anus,explore the timing of this surgery.Methods:There are 132 children with Hirschsprung's disease treated by transanal endorectal single-stage definitive operation,their case data and defecation function follow-up data was analyzed by us.According to the age of surgery is divided into two groups,the neonatal group and non-neonate group.Results:The preoperative time in the neonatal group was shorter than the non-neonatal group.And the anesthesia time and operation time are also shorter than the non-neonatal group[2.99±0.79vs3.41±0.82 hours,p=0.004],[1.96±0.97vs2.36±0.80 hours,p=0.01],length of resected bowel was shorter [31.26±12.20vs41.22±14.39 cm,p=0.003].Postoperative complications in the neonatal group had 7 cases of enterocolitis,and five cases of enterocolitis,three cases of anastomotic leakage,one case of intestinal obstruction,the two groups have no significant difference.Allpatients were followed up for 6 months to 3 years,no statistically differences in the excellent rates and the defecation function between the two groups among three years.Conclusion:Single-stage definitive operation performed in neonatal period have much more advantages compare to the age more than three years,such as the perioperative preparation time,operation time,and anesthesia time are all shorter,resection of the bowel is less,and they can reached good defecation function,So we advocate the surgery can be performed in neonatal period,if HD is confirmed.
