Background:Shock is an acute disease that occurs more often in patients with circulatory failure.According to different pathophysiological basis,it can be divided into four types:hypovolemic shock,cardiogenic shock,obstructive shock,and distribution shock.Timely correction of shock can significantly improve patient mortality.Early diagnosis and etiological screening of shock patients are the key to guide the direction of treatment.Assessment of the patient's volume status and volume response is the basis for ensuring effective fluid therapy.Capacity status and volume response monitoring methods can be roughly divided into invasive and non-invasive types.Invasive monitoring methods such as pulmonary artery catheterization,transcardiography,and CVP are the methods used earlier in the clinical assessment of volume status.However,due to invasiveness and the acquisition of parameters for a certain period of time,the clinical application is limited.In recent years,ultrasonic examination has attracted more and more attention in intensive medical applications because of its characteristics of real-time,non-invasive,repeatable,and timely acquisition of parameters.It has become the preferred method for identifying the type of shock and assessing the state and volume response of shock patients..Ultrasound mainly through the heart,inferior vena cava to determine the type of shock,guide the best treatment program and assess the treatment effect,because of the above-mentioned test is affected by the operator's technique and the patient's obesity,thoracic and abdominal pressure changes results of interpretation differences.In recent years,some scholars have proposed to monitor the volume state through peripheral vascular by ultrasonography,the carotid artery has received more attention because of its superficial position and easy access to the ultrasonic section.Related studies have shown that the correct blood flow in carotid artery can be used for assessing capacity status.Objective:To investigate the influential factors of Corrected Flow Time(FTc)in carotid artery and determine the range of carotid FTc in normal adults.Methods:Carotid ultrasound was performed on 443 adults.The blood flow time(FT),cardiac cycle,heart rate,and carotid artery resistance index were measured by the carotid artery spectrum.the age,sex,height,weight,waist circumference,blood pressure,glucose and lipids were recorded.The FTc,body mass index(BMI)and surface area(BAS)of each paticipant were calculated.199 adults were excluded from incomplete data,infection,abnormal liver and kidney function,abnormal electrocardiogram,anemia,splenomegaly,total 234 adults were included in the study and discussion.Pearson correlation test and multivariate linear regression analysis were used to investigate the probable association between the above factors and FTc,independent sample t test to investigate the difference in FTc between patients with abnormal blood pressure and blood glucose and normal adults.Results:234 adults(127 males and 107 females)were included in the study.The average age was 40.18±9.44 years.There were 37 blood pressure abnormalities,14 abnormal blood glucose levels,and 118 dyslipidemias.(Normal adults are those who without abnormal blood pressure and glucose,a total of 183 adults).Pearson correlation analysis of normal adult carotid FTc with gender,age,height,weight,waist circumference,BMI,BSA,blood lipids,blood glucose,MAP,RI,HR(P values<0.001,<0.001,<0.001,<0.001,<0.001,0.009,<0.001,0.618,0.430,0.119,0.014,0.852).The results showed that the carotid FTc was related to gender,age,increase,quality,waist circumference,BMI,BSA,and RI.The influencing factors of normal adult carotid artery FTC were further analyzed by multivariate linear regression analysis.Multiple regression equations were obtained:FTc(ms)=301.459+7.884Xgender+0.276×age;There was significant difference in FTc of carotid artery between those with abnormal blood pressure and normal subjects(t=2.298,P=0.023);There was no significant difference in FTc of carotid artery between those with abnormal blood glucose and those with normal subjects(t=1.852,P=0.065).The mean FTc of female aged 18-32,33-48,49-69 were 328.77±11.08ms,330.65±14.39ms,and 335.14±12.18ms.The FTc of males aged 18-32,33-48,49-69 were 320.13±13.11ms and 320.90±14.04ms,332.18±11.44ms.Conclusion:Sex and age are influential factors of carotid FTc,there is a statistical difference between carotid FTC and normal in those with abnormal blood pressure.The FTc values of carotid arteries measured in normal adults of different genders can provide reference for clinical assessment of vascular volume.
