Influencing Relapsing Factors Of Infantile Spasms Controlled By Dexamethasone

Objective: To analyze the influencing relapsing factors of infantile spasms(ISs)controlled by dexamethasone;Methods: Fifty-nine cases were collected in the Children's Hospital Affiliated to Chongqing Medical University from August 2009 to September 2016,whose cessation of spasms was at least 28 days.The follow-up of relapsed patients were performed until spasms relapsed.The patients who had no recurrence were followed at least nine months.Gender,age of onset,course of diseases,etiology,the type of electroencephalogram,the time to be controlled by dexamethasone were selected according to literature review,then Logistic regression was used to analyze the relationship between the risk factors and the relapse of spasms;Results: 1.The recurrence rate at 9 months were 33.9%(20/59 cases);2.Age of onset,course of diseases and the risk factors of perinatal period were closely related to the recurrence,the non-classic(early-onset and late-onset)ISs(8/13 cases,61.5%)were more likely to relapse than the classic onset ISs(12/46 cases,26.1%)(?2=4.213,P=0.040),the recurrence rate of the course of diseases within 1 month(2/19 cases,10.5%)was lower than that beyond 1 month(18/40 cases,45%)(?2=6.832,P=0.009),those who had the risk factors of perinatal period(14/30 cases,46.7%)were more likely to relapse than those who without risk factors(6/29 cases,20.7%)(?2=4.441,P=0.035).There were significant differences between two groups(P<0.05);3.Logistic regression analysis demonstrated that course of diseases and the risk factors of perinatal period were the independent risk factors of the recurrence of ISs;Conclusion: Age of onset,course of diseases and the risk factors of perinatal period were closely related to the recurrence,course of diseases and the risk factors of perinatal period were the independent risk factors of the recurrence of ISs.Therefore,early diagnosis and early treatment could improve the curative effect of ISs,reducing recurrence and improving prognosis,meanwhile,the recurrence can be judged by the risk factors of perinatal period.
