Objective:To explore the value of ultrasonography in the diagnosis of epiphyseal injury in children.Methods:48 cases of joint trauma were examined by ultrasonography and 45 of them were classified and measured.In 23 cases of the operative group,18 cases were examined by X-ray,CT and ultrasound.Moreover the results of X-ray,CT and ultrasound were compared with surgical results.The ultrasonic measurement was compared with the CT measurement in 22 cases.Ultrasound typing and surgical typing were all classified according to Salter-Harris classification.Result:The positive incidence of examinations by ultrasound and X-ray were respectively 93.7% and 89.5% among the 48 cases,and the difference between them was statistically significant(P<0.05).In 45 cases of ultrasonic quantitative evaluation,the average ESD of the operative group(n=22)and the non-operative group(n=23)were(7.68±3.35)mm and(2.29±1.38)mm respectively,and there was a significant difference between the two groups(P<0.05).The average measured value of examinations by ultrasound and CT were(6.41±3.86)mm and(5.88±3.71)mm respectively in the 22 cases,and there was no significant statistical difference between them(P>0.05).But there was a significant correlation between the two groups,ultrasound and CT(P<0.05).The ultrasonic typing and the surgical typing matched well among the 23 cases,and there was no significant statistical difference between them(P>0.05).In 23 cases of the operative group,18 cases were examined by CT.There was no significant statistical difference in the positive coincidence rate among examinations by X-ray,CT and ultrasound in 18 cases of the operative group(P>0.05).There was no significant statistical difference of ESD among the different sides and different gender in 45 cases(P > 0.05).Conclusion: The detection rate of ultrasound for epiphyseal injury with smaller age and children with slightly shifted epiphyseal injury is higher than that of X-ray.At the same time,ultrasound can be used as a quantitative method to evaluate the separation of epiphyseal injury,and the classification diagnosis of epiphyseal injury by ultrasound is exact.Therefor ultrasound has certain value in assisting clinical diagnosis and guiding treatment. |
