Objective: To compare the clinical features of full-term infants with respiratory distress syndrome among different grading groups,and to explore the similarities and differences of perinatal factors diagnosis and treatment,prognosis in order to promote the early understanding and diagnosis of full-term neonates with respiratory distress syndrome in clinical practice.Methods: A total of 167 cases of full-term infants with RDS in our department from Dec.2012 to Aug.2017 were divided into four groups according to chest X-ray signs.The clinical data in each group were analyzed retrospectively.Results: RDS 1 group had 10 cases,RDS 2 group had 40 cases,RDS 3 group had 96 cases,RDS 4 group had 21 cases.There was no statistically significant difference in maternal age,gestational diabetes mellitus,way of fertilization,mode of delivery,twins,premature rupture of membranes,placental abruption,gender and gestational age(P>0.05);When the four groups were compared in the birthweight,RDS 1 group > RDS 2 group > RDS 3 group > RDS 4 group;While comparing the proportion of infants with low Apgar score: RDS 1 group< RDS 2 group< RDS 3 group<RDS 4 group,which are of statistically significant difference(p<0.05).The proportion of mechanical ventilation cases had statistically significant difference(p<0.05),when comparisons were made between each group,it showed that the proportion of mechanical ventilation in RDS 4 group was higher than that of RDS 2 group,and the difference was statistically significant(?2=7.663,p=0.006< 0.05);The proportion of mechanical ventilation infants in RDS 4 group was higher than that in RDS 3 group,the difference was statistically significant(?2=6.892,p=0.009<0.05).The improve and cure rate decreased among the four groups: RDS 1 group > RDS 2 group > RDS 3 group > RDS 4 group(p<0.05).In addition,the length of hospital stay prolonged: RDS 1 group< RDS 2 group< RDS 3 group<RDS 4 group,which showed statistically significant difference(p<0.05).Conclusion: 1.There was no significant difference in male,caesarean section,high maternal age,gestational diabetes mellitus,premature rupture of membranes,placental abruption,diagnostic time,starting time of using PS,reuse of PS,dosage of PS,time(length)of mechanical ventilation and the incidence of complications such as intracranial hemorrhage,patent ductus arteriosus,pulmonary hypertension,pulmonary hemorrhage,pulmonary air leakage and bronchopulmonary dysplasia among four groups.2.The grade of RDS in full-term infants is mainly related to the factors of birth mass and asphyxia,the heavier the chest film grading,the longer the hospitalization days,the worse the treatment effect.
