Objective: To measure periprostatic fat volume(PFV)and total prostate volume(TPV)on magnetic resonance imaging(MRI),and calculate the periprostatic fat ratio(periprostatic fat volume to prostate volume,P/T),and to evaluate the correlation between P/T and aggressiveness of prostate cancer.Methods: Eighty patients with prostate cancer diagnosed in our hospital were retrospectively analyzed.According to Gleason score,patients with prostate cancer were divided into two groups: low grade(GS=6)and high grade(GS?7).Meanwhile,30 cases of non-prostate cancer were selected as control group.The subcutaneous fat thickness(SFT),PFV and TPV were measured on the MRI images,and P/T was calculated.And collect clinical pathological information of the subject.The differences in fat measurement between the prostate cancer group and the control group were compared.The correlation between fat measurement and body mass index(BMI)and Gleason score was analyzed,and the independent risk factors for high-grade prostate cancer were obtained.Results:1.The prostate cancer group P/T and the first diagnosis of serum prostate specific antigen(PSA)were higher than the control group(P<0.01),the high-level group P/T,PSA was higher than the low-grade group(P<0.01).There was no significant difference in SFT,BMI,and age between groups(P>0.05).2.Gleason Score was positively correlated with P/T(r=0.561,P=0.000)and PSA(r=0.532,P=0.000).Gleason Score had no significant correlation with age,BMI and SFT.3.Multivariate Logistic regression analysis,P/T was one of the independent risk factors for high grade prostate cancer(P=0.02,OR:1.371).4.There was a positive correlation between SFT and BMI(r=0.645,P=0.000),but there was no significant correlation between PFV,P/T and BMI(P>0.05).Conclusion:The increase of periprostatic fat ratio(P/T)is correlated with the aggressiveness of prostate cancer.P/T is an independent risk factor for high-grade prostate cancer.MRI can predict the malignancy of prostate cancer by measuring the periprostatic fat,and provide advice for prognosis.
