| Objective: The incidence of type 2 diabetic mellitus(T2DM)become higher year by year.Hyperuricemia have attracted more and more attention which was often combined with T2 DM.However,to date data of female T2 DM hyperuricemia remains insufficient,the present study was to discuss relationship between uric acid level of female T2 DM and insulin resistance.All 152 female patients and 68 healthy female volunteers were recruited the study.Uric acid,insulin resistance index and islet β cell function were all detected for reveal relationship between uric acid level and insulin resistance and provide proof for earlier therapy of femal T2 DM combined with hyperuricemia.Methods: A total of 152 women with type 2 diabetes who was in hospital of endocrinology department between September of 2016 and November of2017 were assigned to T2 DM group,and 68 healthy women became to healthy examination at the same period were assigned to normal control group(NC gruoup).Samples of elbow venous blood were collected after 8 to 12 hours fasting.Fasting blood glucose,blood lipids,renal function,uric acid and other biochemical indexes were all detected with fully automatic biochemical analyzer.The glycosylated hemoglobin was measured with electrochemiluminescence method,and the levels of urinary albumin and urine microalbumin were measured with electrochemical immunoluminescence method.All subjects underwent a 75 g oral glucose tolerance test and useful factors were measured.The T2 DM group was divided into four groups according to uric acid levels for further comparison.All data were analyzed with SPSS21.0 software.Results:1.Factors T2 DM group and NC groupThere were no significant differences in age,systolic pressure and diastolic pressure between the two groups.BMI of T2 DM group was significantly higher compared to NC group(P<0.01).The T2 DM group had higher levels of glycosylated hemoglobin,fasting blood glucose,HOMA-IR,urea nitrogen,triglyceride and low-density lipoprotein than NC group(P<0.05),but lower levels of fasting insulin,HOMA-β,and high density lipoprotein(P<0.01).2.Further comparation of four subgroups of T2 DM groupThere was no differences in disease duration,levels of Hb A1 c,creatinine,total cholesterol,LDL,fasting blood glucose,systolic blood pressure,diastolic blood pressure,urea nitrogen and urine microalbumin among the four subgroups.The four subgroups had different levels of age,BMI,triglyceride,24-hour urinary protein,fasting insulin,HOMA-IR,HOMA-β,first phase insulin secretion and second phase insulin secretion(P<0.05).Among them,BMI,triglycerides,fasting insulin,HOMA-β,first phase and second phase insulin secretion levels increased with increasing uric acid levels.When the hyperuricemia diagnosis level was reached,the first phase and second phase insulin secretion decreased slightly.3.Correlation analyses of uric acid and other indicators in T2 DM groupThe uric acid level of T2 DM group has positive relationships with BMI,triglyceride,24-hour urinary protein,urinary microalbumin,HOMA-IR,HOMA-β,fasting insulin,first phase insulin secretion,and second phase insulin secretion,but has negative relationship with high density lipoprotein.There were no relationships between uric acid and disease duration,levels of glycosylated hemoglobin,creatinine,total cholesterol,low density lipoprotein,fasting blood glucose and urea nitrogen.Conclusions:1.Serum uric acid levels was positively correlated with insulin resistance in type 2 diabetes women.2.First phase insulin secretion in type 2 diabetes women gradually increased with serum uric acid levels,but when it reached 360 μmol/L(thediagnostic criteria for female hyperuricemia),first phase insulin secretion was significantly decreased,suggesting that women T2 DM with hyperuricemia,insulin β cell function decreased.3.In type 2 diabetes women,24-hour urinary protein,urinary microalbumin and serum uric acid were positive correlated with level of serum uric acid,which suggested that uric acid was associated with renal function decline. |
