Objective:In patients with type 2 diabetes mellitus(T2DM),the overall prevalence of non-alcoholic fatty liver disease(NAFLD)is significantly higher.Foreign studies have shown that 61% of diabetic patients have hepatic steatosis,of which 53% are obese,and more Many scholars believe that NAFLD may be one of the early complications of T2 DM.Sortilin-1(SORT1)is a type I membrane glycoprotein encoded by the SORT1 gene and plays an important role in the intracellular and extracellular secretion,transport,and signal transduction.SORT1 may be involved in the regulation of liver lipid metabolism and lipid metabolism caused by insulin resistance.The purpose of this study was to investigate the role of SORT1 in the development of NAFLD in patients with T2 DM by testing serum SORT1 levels and hepatic fat content in T2 DM patients.Methods:124 subjects were selected and classified into T2 DM group,NAFLD group,and T2DM+NAFLD group by ultrasound quantitative liver fat content.40 healthy volunteers were also selected as normal control group.All subjects recorded clinical data such as height,weight,hypoglycemic treatment,diabetes duration,and fasting blood glucose(FBG),fasting insulin(FINS),glycosylated hemoglobin(Hb A1c),triglyceride(TG),and total cholesterol(TC).Low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),alanine aminotransferase(ALT),glutamyl transpeptidase(GGT),and serum SORT1 levels.Comparing SORT1 levels and related indicators between the subjects in each group were compared for significant differences,and the correlation between SORT1 and related parameters was analyzed.Result:Serum SORT1 levels in T2 DM group,NAFLD group and T2DM+NAFLD group were lower than those in control group(all P<0.05).Serum SORT1 levels in T2DM+NAFLD group were lower than those in T2 DM group and NAFLD group(all P<0.05),but T2 DM group.There was no significant difference in SORT level between the NAFLD group and the NAFLD group(P>0.05).Compared with the control group,the liver fat content in NAFLD group and T2DM+NAFLD group was higher than that in the control group,and the difference was statistically significant(all P<0.05).Compared with the control group,the levels of TG,FBG,HbA1 c,and HOMA-IR in the three groups were significantly higher(all P<0.05).Correlation analysis showed that SORT1 and BMI,FBG,HbA1 c,HOMA-IR and Liver fat content(r =-0.243,r =-0.331,r =-0.308,r =-0.375,r =-0.337,all P <0.05)was negatively correlated.Multiple linear stepwise regression analysis showed that HbA1 c is independent of SORT1 Influencing factors.Logistic regression analysis showed that SORT1 is a protective factor for type 2 diabetes with non-alcoholic fatty liver disease.Conclude:Compared with patients with type 2 diabetes and non-alcoholic fatty liver disease,serum SORT1 levels in patients with type 2 diabetes complicated by non-alcoholic fatty liver disease were reduced.SORT1 is a protective factor for type 2 diabetes with non-alcoholic fatty liver disease.
