Objective To investigate the diagnosis of cerebellar hemorrhage in premature infants based on the combination of conventional MRITIWI / T2 WI and DWI,and to analyze the incidence of cerebellar hemorrhage and its distribution in all groups of gestational age and body weight at birth.To investigate the risk factors of cerebellar hemorrhage in premature infants and its correlation with other types of brain injury.Methods A total of 2668 premature infants(< 37 weeks)hospitalized in our hospital from January 2008 to December 2015 who underwent head MRI examination(< 37 weeks)were evaluated according to the criteria of brain injury on the classification statistics and to analysis to its general situation.Results(1).Of total 2668 premature infants,brain injuries found in 920.The incidence of WMD was 27.59%(736/2668),IVH was 8.73%(233/2668),WMD with IVH 2.70%(72/2668).The incidence of CH was 2%(54/26682),35 cases of unilateral involvement(64.8%).The incidences of CH among the different groups of gestational age and body weight has not been shown remarkbling significance.Of 54 cases 17 with simple CH and 36 with combination with WMD and IVH,but for only one with brain abscess.The incidence of CH in WMD+IVH group(4.01%)is higher than that in the non-WMD+ IVH group(0.96%)(?2=28.513,P=0.0001).The incidence of simple IVH in CH group and non-Ch group was 18.52% and 5.78% respectively,with significant difference between them.The incidence of WMD+ IVH in CH group and non-Ch group has also been shown statistically significant(?2=52.531,P=0.0001).(3).The distribution and incidence of birth weight and pulmonary hemorrhage in grade?-?CH group and grade ?-? were significantly different(P < 0.05).The incidence of WMD,IVH(?-?)in?-?degree and ?-? degree was statistically significant(P<0.05).(4).Of the 54 cases CH,simple CH accounted for 31.48%(17),only with WMD 29.63%(16)and IVH 18.52%(10).Of them,10 cases of CH were accompanied by WMD and IVH(18.52%).On the basis of group CH,the factors of each group were compared.There has been shown the significant difference on premature rupture of membranes,body weight and gestational age between CH and CH+ WMD groups(P < 0.05);It has been found that the difference on infection and weight distribution between CH group and CH+IVH group had statistical significance(P < 0.05);The incidence of pregnancy induced hypertension(PIH)and noninvasive ventilation in CH+ WMD+IVH group was significantly higher than that in CH group(P < 0.05).Conclusion In preterm infants the incidence of cerebellar hemorrhage is relatively lower with independence of gestational age and body weight at birth,often combined with other types of brain damage.Under the condition of high risk factors such as pulmonary hemorrhage,mechanical ventilation and infection,there are more likely to occur to the CH with combination of WMD and IVH which may be more severe.
