Objective:Aim to Analyse the etiology and antibiotic susceptibility in severe pneumonia,and guide rational drug use in clinic.Methods:The sputum samples?blood specimen?bronchoalveolar lavage fluid and pleural effusion of265 cases with pathogenic microorganism culture and drug sensitivity were performed from 2014 to2016 in our hospital.The Excel software was used to analyze the screening data and the SPSS 19 was used for statistical analysis to obtain the drug resistance of the bacteria to the commonly used antibiotics.Results:There were 155 positive cases from pathogenic microorganism culture among 265 patients,the positive rate was 58.49%.There were 238(36.67%)positive cases from 649 case sputum culture,bacterium are 176 cases ? The positive rate of the top three is 78 strains of Acinetobacter baumannii(n = 44.31%)?50 strains of Pseudomonas aeruginosa(28.41%)? 17 strains of Klebsiella pneumoniae(9.66%).There were 77(12.90%)positive cases from 597 case blood culture,bacterium are73 cases.The positive rate of the top four is 14 strains of Klebsiella pneumoniae(19.18%),8 strains of Pseudomonas aeruginosa(10.96%),8 strains of Staphylococcus aureus(10.96%),7 strains of Bauman Acinetobacter(9.59%).There were 3(9.68%)positive cases from 31 case pleural effusion culture,which had 2 strains of Acinetobacter baumannii and 1 strain of Staphylococcus aureus.There were 8 case of bronchoalveolar lavage fluid culture,which had 1 strain of Pseudomonas aeruginosa.The resistance rate of Acinetobacter baumannii and Pseudomonas aeruginosa was high,other pathogens also show various degrees of tolerance to antibiotic.Conclusion:The pathogenic bacteria of severe pneumonia were mainly Gram-negative bacteria,and Gram-positive bacteria were mainly Staphylococcus.The drug resistance of pathogenic microorganisms in our hospital was generally higher than the national average.The drug resistance of Pseudomonas aeruginosa to tobramycin increased year by year,The drug resistance of Acinetobacter Bauman to Amikacin and tegocycline decreased year by year.The clinical selection of antibiotics should be combined with the disease characteristics of the patients in our hospital...
