Objective:Conducting a retrospective study to explore the value of 18F-FDG PET-CT metabolic parameters in predicting the status of epidermal growth factor receptor(EGFR)mutations in non-small-cell lung cancer(NSCLC)patients.The target metabolic parameters include maximum standardized uptake value(SUVmax),mean standardized uptake value(SUVmean),metabolic tumor volume(MTV)and total lesion glycolysis(TLG).Methods:We collected clinical information of 92 patients diagnosed of lung adenocarcinoma who underwent PET-CT screening and had definite pathological diagnosis and EGFR mutation status before accepting anti-tumor therapies.Receriver operating characteristic(ROC)curve analysis and was then used to validate the predictive value and obtain the cut-off value and area under curve(AUC)of SUVmax,SUVmean,MTV and TLG(AUC>0.5 suggests good discrimination).A multivariate logistic regression analysis was used to validate the association between PET-CT metabolic parameters and EGFR mutations.Results:Among 92 lung adenocarcinoma patients,48(52%,48/92)had EGFR mutations.ROC curve analysis shows:the optimal critical value of SUVmax?SUVmean?MTV and TLG are 9.63(AUC=0.684,95%CI=0.576-0.793,P=0.002),5.71(AUC=0.661,95%CI=0.550-0.772,P=0.008),14.69cm~3(AUC=0.563,95%CI=0.443-0.682,P<0.001),77.93(AUC=0.613,95%CI=0.497-0.7292,P=0.059).Multivariate analysis revealed that no smoking history(P=0.006)and low levels of SUVmax(P=0.021)were predictive of haboring EGFR mutation.Conclusions:SUVmax of 18F-FDG PET-CT is an independent factor for predicting EGFR mutation in patients of lung adenocarcinoma,with certain reference value in predicting EGFR mutation status.
