Objective: To analyze clinical characteristics of diagnosis,treatment and prognosis in octogenarians with abdominal aortic aneurysm(AAA).Methods: Follow-up visits and clinical analysis were conducted from January1 2004 to May31 2016 on number of 57 AAA patients aged over 80 years old and 154 patients aged below 80 years old who had been treated with endovascular aneurysm repair(EVAR)and thus met the inclusion criteria.The Kaplan-Meier survival curve was used to analyze the medium-and long-term survival rate in patients who had been treated with EVAR,and survival rate was analyzed using the Log-rank test.Results: Effective follow-up visits were conducted on 40 octogenarian AAAs and 131 relatively young AAAs who had been treated with EVAR.In the octogenarian AAAs,there was no significant difference between the two treatments EVAR and conservative treatment in the hospital,medium-and long-term survival rate(P>0.05),and the hospital expenses of EVAR were significantly higher than open surgery repair(OSR)(P<0.05).The incidence of hypertension for the octogenarian AAAs is higher than the relatively young AAAs.In the matter of the maximal diameter of aneurysm,the proportion of AAAs that aneurysm diameter larger than 50 mm and being complicated with one side of iliac artery occlusion,the octogenarian AAAs were significantly higher than the relatively young AAAs(P<0.05).The octogenarian AAAs had significantly greater infrarenal neck angulation(P<0.05).The time for out-off-bed activity in the octogenarian AAAs was longer than the relatively young AAAs with EVAR(P<0.05).The incidence of type I endoleak and re-intervention of the octogenarian AAAs were significantly higher than the relatively young AAAs(P<0.05).The octogenarian AAAs were significantly higher than the relatively young AAAs in 3-,5-year survival rates(P<0.05).Conclusion: Agedness is an important factor affecting the prognosis of AAAs.EVAR should be used as the treatment of the octogenarian AAAs preferred,but compared with the treatment of relatively young AAAs,EVAR still has more complications.
