A Clinical Analysis Of 21 Patients With Monocyte Listeria Sepsis

Objective:.To identify the clinical features of the 21 patients from the first hospital affiliated to China Medical University with listeria septicaemia in recent 5 years,and to improve the diagnostic level of the disease.Methods:The clinic data of 21 inpatients who were diagnosed with listeria sepsis in the First Affiliated Hospital of China Me-ical University from 2012 to 2017 were collected and reviewed retrospec-tively.The clinical data including general information,past medical history,clinical manifestation,laboratory examination,physical examination,before and after the hospital diagnosis,diagnosis,treatment,prognosis and time first department etc.Results:Among the 21 cases of listeria septicemia diagnosed as mononuclear cells,most of them were female.The age between 1 day and 81 years old,with newborn,middle age and old age patients.The department was mainly focused on neonatal(33.34%)and rheumatoid immunology(33.34%).9 cases(42.86%)showed fever with neck rigidity and consciousness disturbance.3 cases(14.29%)showed fever with cough,expectoration and lung wet rales.2 cases(9.52%)showed fever with nausea,vomiting,abdominal pain or diarrhea;1 case(4.76%)showed a headache,and 1(4.76%)showed a fever accompanied by a rash,and 1(4.76%)showed a fever accompanied by a pain in the liver area.The blood routine increased with the total number of leukocytes,the neutrophils ratio increased,the monocyte ratio was normal,the hemoglobin normal,the platelet was normal.The inflammatory indexes were mainly elevated with CRP and PCT.Cerebrospinal fluid examination: total protein increased obviously;The number of cells increased significantly,glucose decreased,and chloride decreased 6 cases.21 cases of patients,18 cases of application of mononucleosis listeria has antibacterial activities of penicillin,piperacillin,sulfonated ampicillin,beauty e.faecalis,levofloxacin,mo xisha magnitude of antibiotics,11 cases improved discharged from hospital.Conclusion:Listeria monocytosis is more likely to occur in people with compromised immune systems,such as newborns,over 65 years of age,and patients with immunosuppressive therapy.Clinical manifestations are mainly fever,about half of meningoencephalitis or merger,show the mind change,and even coma,often life-threatening illness serious,may also be associated with infection in areas such as the respiratory system,digestive system.The blood routine was increased mainly by the total number of leukocytes and neutrophils,and the increase of CRP and PCT was confirmed by the positive blood and aseptic humoral culture.Early application of effective antibiotics can improve cure rate and improve prognosis.Remind clinicians do more blood in patients with suspected diagnosis of the disease and aseptic humoral bacterial culture,treatment as soon as possible to choose penicillin,ampicillin,and beauty e.faecalis sensitive drug resistance anti-infection therapy.
