Background and objective: Intracranial aneurysm is the most common cause of subarachnoid hemorrhage,intracranial aneurysm without rupture can cause less obvious symptoms(unruptured aneurysm of posterior communicating artery can cause oculomotor nerve palsy,giant unruptured aneurysms can cause headaches and other symptoms of intracranial hypertension),and because of the rapid development after the onset of illness,so that etiology and pathophysiology of intracranial aneurysms is not completely clear at present,therefore intracranial aneurysm model is helpful for us to further understand the etiology and pathogenesis of intracranial aneurysms.The treatment of intracranial aneurysm has been developed,mainly including clipping and endovascular embolization,especially the rapid development of intravascular interventional therapy in the treatment of intracranial aneurysms has been able to have a good effect,but at this stage there are still some complex intracranial aneurysm with poor treatment,and intracranial aneurysm model for the study of the treatment of intracranial aneurysms still plays an irreplaceable role,especially for the essential research of new equipment.Now the intracranial aneurysms in vivo animal models mainly include dogs,rabbits,rodents,in addition to some rare models of pig and chimpanzee;the commonly used aneurysm models include external jugular vein graft to the carotid artery of the canine,and elastase induced aneurysm model of right common carotid artery of rabbit,and elastase induced aneurysm at intracranial Willis ring of hypertension-mice.Dogs and rabbits models mainly used for the research of the treatment at intracranial aneurysms;the murine models mainly used to explore the pathogenesis of intracranial aneurysms.This experiment is aimed at establishing a complex intracranial aneurysm model at a rabbit which can make a elastase induced bifurcation aneurysm model that can be applied to explore the interventional treatment of complex intracranial aneurysmsMethods: The 10 New Zealand white rabbits,at supin position,after anesthesia that via ear marginal vein,applicated surgical method,using micromanipulation instrument and head mounted magnifying glass that attach a heeadlight,were separated the right carotid artery to the left common carotid artery starting,(Department of rabbit brachiocephalic trunk first into the left common carotid artery,and then divided into the right carotid artery and the right subclavian artery),the right carotid artery was ligated above 2CM of the onset,temporary occlusion of the right subclavian artery and brachiocephalic trunk artery(after the separation of the left common carotid artery),injection of elastin enzyme in that closed lacuna,20 minutes later,remove blocking,and suture.Random sampling of one rabbit was injected of normal saline into the closed space as control.Three weeks after operation the cervical aneurysm were in progress of anatomic and pathological analysis.Results: The aneurysm model was successfully made in 8 of 10 rabbits.The right carotid artery was dilated,the elastic layer was disordered and decreased.The anatomy of a rabbit without elastase showed the stump of the right internal carotid artery,no obvious aneurysmal dilatation,and pathological examination showed that the elastic layer was arranged neatly,and the elastic layer did not significantly reduce.One rabbit was dead during the operation.,the cause of death was pneumothorax.Conclusion: The temporary occlusion of brachiocephalic artery and right subclavian artery,and ligation of right CCA,thus forming a closed lacuna which injected porcine elastase can successfully establish a bifurcation aneurysm model.
