Clinical Analysis Of Extracorporeal Membrane Oxygenation In The Treatment Of Patients With Cardiopulmonary Failure

Objective:To analyze the data of ECMO in the treatment of cardiopulmonary failure and summarize the clinical experience in order to improve the effect of ECMO treatment.Methods:19 patients with cardiopulmonary failure treated by ECMO in our hospital from January 2015 to December 2017 were collected.They were divided into survival and death groups to analyze clinical data.Results:Among the 19 patients,6 patients?31.8%?survived and 13 patients?62.8%?died.There was no statistical difference in the data of gender,age,weight,STB,ALT,Cr,WBC,Hb,Hct,PT,APTT,Fib,OT,CPBT and ACCT before ECMO was assisted?P>0.05?.There was statistical difference in the data of blood lactate,mean arterial pressure?dosage of dopamine or epinephrine or noepinephrine before ECMO was assisted?P<0.05?.Assistant time in the survival group was 115.3±66.7h and the longest time was 195.2h.Assistant time in the dead group was 132.2±59.4h and the longest time was 237.8h.There was statistical difference in ALT in the first and second day after ECMO was assisted?P<0.05?.There was statistical difference in Cr in the 5th day and urine volume in the 3rd to 5th day.12 patients?63.2%?had complications in bleeding,11 patients?57.9%?were diagnosed with renal insufficiency,4 patients?21.0%?were diagnosed with disseminated intravascular coagulation,2 patients?10.5%?were diagnosed with lung infection,1 patient?5.3%?was diagnosed as lung and blood infection,1 patient?5.3%?was diagnosed with subarachnoid hemorrhage,and 1 patient?5.3%?was diagnosed with extremity-ischemia.Conclusion:ECMO is an important treatment method for patients with cardiopulmonary failure caused by LCOS,myocarditis and severe pneumonia.The levels of lactate,dosage of dopamine or epinephrine or noepinephrine before ECMO was assisted,were related to the patients'survival.The levels of lactate,renal insufficiency,urine volume,PaO₂,central venous pressure after ECMO was assisted,were related to the patients'survival.ALT might be related to the patients'survival.
