| Objectice Making a comparison of effects of continuous glucose monitoring system(CGMS)continuous subcutaneous insulin infusion(CSII)(3Cpump),and a retrospective continuous glucose monitoring system combined CSII(2C plan)respectively in treatment of both refractory and newly diagnosed T2 DM,and an assessment of the efficiency and safety of the two treatment options,this paper is to provide individualized treatment experience for clinicians as well as scientific evidence for optimizing the therapy of diabetes.Methods 1 from the patients that the Endocrinology Department of Tangshan gongren Hospital and Tangshan eighth Hospital received during the period from November 2014 to November 2017,39 patients with refractory T2 DM and 51 with newly diagnosed T2 DM were selected,then each type of patients were further randomly divided into two groups,labeled as Group 2C and Group 3C respectively.2 Before treatment,lectures on diabetes were delivered by professionals to ensure that all patients' knowledge on diabetes is at a comparable level.3C Group received treatment of 722 real-time dynamic glucose monitoring system.with monitoring,performed with the high or low glucose alarm on,while Group 2C received treatment of 508 C insulin with continuous glucose monitoring system,and both groups took a test on blood sugar of finger the calibration values of which are input.For group 3C,the values of Hyperglycemia and Hypoglycemia being set up,under the circumstances of a big blood glucose fluctuation or an alarm on blood glucose,doctors gave timely medical treatment,and thence,on the basis of the analysis via Care link,an overall adjustment was made with respect to diet,exercise,adjustment,insulin plan.While,for Group 2C,a daily blood sugar map was exported and according to it,an adjustment was made with respect to diet,exercise,and insulin plan,and The two groups were all using mentodon insulin.The initial insulin dose calculation was consistent,After 6 days of treatment,the two groups were treated with 4 times daily subcutaneous insulin injection.3 Some indicators were compared through six days of observation.(fasting blood glucose,postprandial blood glucose,average blood sugar in daily,mean fluctuations extent of blood glucose,average blood sugar in daily,the frequency of hyperglycemia,the time required for blood sugar to reach the target,the insulin dosage,the time of education for patients during the period of pump,the number of patients' counselling,and the number of alerts to be handled).4 The SPSS18.0 software was used for analysis,and t-test was used for comparison between the two groups,the paired T test was used for comparison between before and after treatment.Chi-square test was used for counting data,and P<0.05 was considered statistically significant.Results The results were the same in two kinds of people,in two groups the fasting blood glucose and postprandial blood sugar after treatment were all significantly decreased(P<0.01),3C treatment group decreased more significantly(P<0.05),two groups people's MBG and MAGE in 6th day significantly less than first day(P<0.01),drop is more obviously in 3 C group(P<0.05),the total number of hypoglycemia in the 3C treatment group was lower than that in the group C group(P<0.05),the time required for blood sugar to reach the target in 3C group is shorter then in double C group(P<0.01),but the times of education with pump,the number of consultation with the pump,and the number of alarms needed to be handled by the medical staff in 3C treatment group are more than those in double C group(P<0.01).Conclusion Of 3C patients with insulin pump significantly reduced fasting and postprandial blood sugar,lowering blood sugar fluctuations and the frequency of hypoglycaemia,shortens the time of blood glucose control is at target and realize the fast,safe and effective hypoglycemic,has high clinical value. |
