Objectives Analysis of different types of liver space occupying lesions by using the sound speed correction?SSC?technique;The clinical value of sound speed correction technique and conventional ultrasonic examination in differential diagnosis of benign and malignant of hepatic solid space occupying lesions;To find the best critical value of the ZSI for differential diagnosis the benign and malignant of hepatic solid space occupying lesions with the sound speed correction technique.Methods Case group:A total of 225 hepatic occupying lesions?193 cases?in the ultrasound department of the Affiliated Hospital of North China University of Science and Technology were selected from October 2016 to February 2018.It was divided into three sub groups:malignant solid occupying lesions 83?the number of hepatocellular carcinoma is 40,hepatic cholangiocarcinoma is 9,metastatic liver cancer is 34?;benign solid occupying lesions 80?the number of hemangioma is 48,the liver abscess of parenchymal phase is 24,the Focal Nodular Hyperplasia is 5?;and the simple hepatic cyst is 62.Control group:92 cases of normal liver tissue around the case group were selected.The parameters collected include:ultrasonic parameters?size,location,patients with subcutaneous layer thickness,occupying the upper edge of the hepatic tissue thickness and the zone speed index?ZSI?of the liver?;the clinical parameters:?age,gender,height,weight and the body mass index?BMI??.Excel 2003 is used to establish a database and the SPSS 17 software is used to analyze the data.The measurement data of normal distribution are expressed byx±s,and the partial distribution data are expressed by M[P25,P75].The comparison of normal distribution data among multiple groups was performed by single factor analysis of variance and the SNK test was selected in the group intercomparsion;The comparison among the measurement data of skewed distribution is done by rank sum test;Comparison of counting data using chi square test.To find the best critical value of ZSI of benign and malignant of hepatic solid space occupying lesions use the receiver operating characteristic?ROC?curve.The test level alpha is 0.05.Results 1 To compare the ZSI values of different types of liver space occupying lesions:1)the ZSI value among malignant solid lesions,benign solid lesions,simple hepatic cyst and the control group is different,and the difference is statistically significant?P<0.05?,The ZSI value from big to small order is malignant solid lesions,benign solid lesions,control group and simple hepatic cyst.This indicates that the hardness of the malignant solid lesions is the first,the benign solid lesions is the second,the normal liver tissue is third and the simple hepatic cyst is the lowest.2)The ZSI value among hepatocellular carcinoma,cholangiocarcinoma,metastatic liver cancer and hepatic hemangioma is different,and the difference is statistically significant?P<0.05?,The ZSI value from big to small order is Cholangiocarcinoma,hepatocellular carcinoma or metastatic liver cancer,and hepatic hemangioma.This indicates that the hardness of the Cholangiocarcinoma is the first,the hepatocellular carcinoma or metastatic liver cancer is the second,and the hepatic hemangioma is the lowest.Meanwhile,there is no significant difference in the hardness between hepatocellular carcinoma and metastatic liver cancer.2 To compare the sound speed correction technique and conventional ultrasonic examination in the differential diagnosis between malignant solid occupying lesions and benign solid occupying lesions on the basis of the gold standard of pathological results:The accuracy of them is eighty-four point six seven percent and seventy-three point six two percent,and the difference is statistically significant?P<0.05?.The accuracy of the sound speed correction technique is higher than that of the conventional ultrasonic examination.This indicates that the sound speed correction technique is superior to the conventional ultrasonic examination.3 To analysis the ZSI value between malignant solid occupying lesions and benign solid occupying lesions by ROC curve.When the value of ZSI was 29m/s,the sensitivity is eighty-nine point two percent,and the specificity is eighty percent.This indicates that the ZSI value of 29m/s can be used as the best critical value for comparing the benign and malignant solid occupying lesions.Conclusions 1 The hardness of malignant solid lesions is higher than that of benign solid lesions and normal liver tissues.The hardness of the cholangiocarcinoma is the first,the hepatocellular carcinoma and metastatic liver cancer is the second,and the hepatic hemangioma is the third.The hardness of the solid space occupying lesions of liver is higher than that of simple hepatic cyst.2 The sound speed correction technique is superior to the conventional ultrasonic examination.3 The sound speed correction technique provides a new quantitative diagnostic reference for the evaluation of the nature of hepatic space occupying lesions.The ZSI value of 29m/s is the best critical value for the differential diagnosis of the benign and malignant hepatic solid space occupying lesions.
