Objective To explore the correlation between blood lipid levels and carotid atherosclerosis in patients with cerebral infarction by using 256-slice spiral CT,and to help clinicians find suitable opportunities for intervention of blood lipid levels.Methods A total of 233 patients with cerebral infarction diagnosed by the Affiliated Hospital of North China University of Technology from September 2015 to September 2016 were selected.Among them,117 cerebral infarction patients with dyslipidemia were classified as the dyslipidemia group,and 116 cerebral infarction patients with normal lipids were the normal blood lipid group.After collecting patients' general clinical data,including gender,age,blood pressure,smoking status and blood glucose,233 carotid angiography were performed on 256-slice spiral CT to observe the incidence of carotid atherosclerosis,carotid lumen stenosis caused by carotid atherosclerotic plaques and the occurrence of various carotid atherosclerosis plaque.The incidence of carotid atherosclerosis,carotid lumen stenosis caused by atherosclerotic plaques,and various atherosclerotic plaques were contrasted respectively.Meanwhile,in line with the degree of hyperlipidemia,the dyslipidemia group was further divided into the elevated blood lipid group and the dyslipidemia group to compare two groups' incidences of carotid atherosclerosis,degree of carotid caused by atherosclerotic plaques,and nature of atherosclerotic plaques.Results Incidences of carotid atherosclerosis in the normal blood lipid group and dyslipidemia group were 84.5% and 95.7%,which difference between the two groups had a statistical significance(?~2=8.277,P=0.004,P<0.05);Incidences of carotid nonstenosis,mild-moderate stenosis and severe stenosis in the normal blood lipid group were 15.5%,67.2% and 17.2% respectively.The occurrence rate of carotid non-stenosis,mildmoderate stenosis and severe stenosis in the dyslipidemia group was 4.3%,67.5% and 28.2% respectively.The difference between the two groups had a statistical significance(Z=-2.967,P=0.003,P<0.05).It indicates that different degrees of carotid stenosis in those two groups are of different component percentages;The incidence of carotid calcified plaques,mixed plaques,and soft plaques in the normal blood lipid group was respectively 65.7%,23.4% and 10.8%,while their incidences in the dyslipidemia group was respectively 51.1%,31.2% and 17.7%.The difference between the two groups was statistically significant(?~2=26.334,P=0.000,P<0.05),indicating that different types of carotid atherosclerosis between the two groups exist different component percentages.Moreover,the incidence of carotid atherosclerosis in the elevated blood lipid group and dyslipidemia group were 93.1% and 98.4%,which was no significant difference between the two groups(Fisher Exact Probability P=0.648,P>0.05).It can be seen that there was no difference in two groups' incidence of carotid atherosclerosis;The incidence of carotid non-stenosis,mild-moderate stenosis,and severe stenosis was 6.9%,48.2% and 44.8% respectively,in the elevated blood lipid group,while their incidences in the dyslipidemia group were 1.6%,44.1% and 54.2%.There was no significant difference between the two groups(Z=-1.228,P=0.219,P>0.05).It means that different types of carotid atherosclerotic plaques were the same;Additionally,incidences of carotid calcified plaques,mixed plaques,and soft plaques in the elevated blood lipid group were respectively 52.2%,30.1% and 16.8%,while their incidences in the dyslipidemia group were 50.1%,31.3% and 18.5%.There was no statistical difference between the two groups(?~2=0.443,P=0.801,P>0.05),indicating that different types of carotid atherosclerotic plaques between the two groups has the same component ratio.Conclusions 1 Compared with normal blood lipids person,the likelihood of atherosclerosis is increased in person with abnormal blood lipid levels;2 Compared with the normal lipid level,abnormalities in blood lipid levels worsen carotid artery stenosis;3 Compared with normal blood lipids person,the possibility of unstable plaques such as mixed patches and soft plaques is increased in patients with abnormal blood lipid levels,and the likelihood of calcified plaques is reduced;4 Effects brought by the elevated blood lipid and the dyslipidemia for the carotid artery are consistent.Therefore,when the marginal increase of blood lipid occurs,clinical attention should be caused and the level of blood lipids should be actively controlled.
