| Objectives By collecting gender,age,body mass index,duration of diabetes and other basic information in patients with type 2 diabetes mellitus,determining the clinical indexes of blood fat,leptin,glycometabolism,and leptin receptor gene Gln223 Arg polymorphism,then explore the lipid metabolism,glucose metabolism,leptin,leptin receptor gene Gln223 Arg polymorphism and diabetes in nonalcoholic fatty liver disease risk.Trough these studies provide new ideas for the prevention,early diagnosis and treatment of nonalcoholic fatty liver disease in patients with type 2 diabetes.Methods We looked at 234 hospitalized type 2 diabetic patients who were from the Department of Endocrinology in the Affiliated Hospital of North China University of Technology,China.The patients were divided into two groups:the control group consisting of 120 patients without NAFLD and the experimental group consisting of114 patients with NAFLD.Record height,weight,duration of diabetes,calculation of BMI,detection of total cholesterol in patients(TC),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol(LDL-C),triglyceride(TG),glycosylated hemoglobin(HbAlc),fasting blood glucose(FPG),fasting insulin(FINS),insulin resistance index(HOMA-IR),leptin,and the polymorphism of gene Glb223 Argin the patients were genotyped by PCR-RFLP.Data entry Excel database,using SPSS17.0software for statistical analysis.Measurement data using the(x±s)said;the average number of the two groups was compared with the independent sample t test;non normal distribution measurement data of the two groups were compared using the Wilcoxon rank sum test;count data by chi square test;using Hardy-Weinberg(H-W)balance test confirmed the population representativeness.The risk factors of type 2 diabetes mellitus combined with non-alcoholic fatty liver disease were analyzed by multivariate Logistic regression analysis.P<0.05 showed statistical significance.Results 1 There was no significant difference in the gender composition between the two groups through the chi square test(P>0.05).2 The age,HDL-C,LDL-C and TC between the two groups showed no statistical difference(P>0.05).3 The BMI,course of disease,HbA1 c,HOMA-IR,leptin and TG in two groups were tested by t test and rank sum test.The results showed that BMI,course of disease,HbA1 c,HOMA-IR,leptin and TG in case group were higher than those in control group,the difference was statistically significant(P<0.05).4 the frequencies of AA,AG and GG genotypes in the control were 31.7%,53.3%and 15.0%respectively;the frequencies of AA,AG and GG genotype in experimental group were 18.4%,33.33%and 48.2%respectively.The frequency of the GG genotype in the experimental group is significantly higher than that in the control(χ~2=30.145,P<0.01).There was also a significant difference in frequency of the G allele,with a higher frequency found in the experimental group(χ~2=25.363,P<0.01).The risk of NAFLD in patients with T2 DM carrying G allele is 2.950 times of patients carrying A alleles.The risk of NAFLD in T2 DM patients carrying AG and GG genotype is 1.074 and 5.529 times higher than those in patients with AA genotype.5 The t test,chi-square test,rank sum test statistically significant variables,introduce to the multi factor non conditional Logistic regression analysis showed that GG genotype can increase the risk of NAFLD inT2 DM patients by a factor of 12.439(95%CI:3.837-40.326).Because the evidence was statistically significant,the GG genotype can be the possible susceptibility factor for NAFLD in type 2 MD patients.It can also be seen from the table that BMI,HbA1 c,Leptin,and the course of diabetes have statistical significance(P<0.05).Conclusions 1 The frequencies of LEPR genotype Gln223 Arg locus GG genotype and G allele in type 2 diabetic mellitus with NAFLD patients were higher than patients who without NAFLD.2 Type 2 diabetic mellitus with NAFLD patients were interrelated with the Gln223 Arg locus GG genotype of the LEPR gene,and the GG genotype can be the susceptibility factor for NAFLD in type 2 MD patients. |
