The implementation of grading diagnosis and treatment policies aims at alleviating the pressure of “overcrowding” in large hospitals,increasing the rate of visiting patients at the grassroots level,allowing basic medical and health institutions to go out of the “difficulty”,establishing new medical order,and promoting “advancement of minor diseases into the community.The formation of a doctor's visit to the hospital for serious illness.However,this Huimin policy has been implemented for many years,but it has yet to achieve a reasonable medical pattern.There are still some urban residents who are reluctant to choose the first clinic at the grassroots level or appear to have a low number of visits to primary medical institutions.This article focuses on the following two aspects: the willingness of the urban residents to receive the first visit(that is,whether urban residents choose to visit a primary medical institution after illness)and the behavior of urban residents in seeking medical care(that is,the number of visits to primary medical institutions by urban residents in the past year).This article uses the survey information of some community city residents in six districts of Xiamen City to select primary-level medical institutions for medical treatment.Firstly,we examined the correlations between individual differences,policy recognition,medical trust,medical habits,medical expenses and medical services,and the willingness of first-time residents in Xiamen urban residents and the behaviors of doctors seeking medical treatment.The factors with insignificant correlation were excluded;On this basis,using binary Logistic regression analysis of the factors affecting the willingness of urban residents for the first visit at the grassroots level;at the same time,using multiple linear regression analysis of the factors affecting urban residents' medical behavior;Finally,on this basis combined with Simon's bounded rationality The theory of decision-making attempts to explain the intrinsic logic that influences the willingness of first-time residents and the behavioral factors of doctors in Xiamen urban residents.The study found that the primary residents' willingness to visit the clinic and the behavior of seeking medical care cannot take into account all possible influencing factors.This is a limited-rational decision-making process and affects the degree of primary residents' willingness to attend the first visit and the degree of hospitalization behavior.There are some differences.Among them,the main factors affecting the willingness of the urban residents at the primary clinic are medical habits and medical trust;the main factors affecting urban residents' medical behavior are medical expenses and medical services.Therefore,in the process of implementing a hierarchical diagnosis and treatment policy and improving the basic medical and health care system,the grassroots government should start from the perspective of urban residents making limited rational medical decisions after illness,and consider that urban residents may be affected when making different medical decisions.What are the main factors affecting the situation and thus grasping the main contradiction of the problem and prescribing the right medicine to effectively alleviate the current situation of the ordinary people who are “seeing the disease and seeing the disease expensively”.
