Objective To determine whether a rise in systolic blood pressure(SBP)?30 mm Hg and/or diastolic blood pressure(DBP)? 15 mm Hg before and after pregnancy is associated with the incidence of preterm and low birth weight infant(LBWI).Method We take women at the reproductive age of Kailuan workers who have taken physical examination and inpatient delivery from 2006 to 2016 as the obserational cohort group(n = 993),divided the objects into four groups according both the diagnose history of Hypertensive Disorders of Pregnancy(HDP)during pregnancy and the net change of blood pressure before and after pregnancy(? BP)? 30/15 mm Hg or not,then compared the incidence of preterm and LBWI between groups,and multivariate logistic regression were used to analyze the effects of the ?BP on the incidences.Results 40 cases(4%)in the cohort were diagnosed as HDP.A total of 187 cases(18.8%)showed Large ?BP.The total incidences of preterm and LBWI in the cohort were 6.9% and 6.2%,respectively.The incidences of preterm in group A1,A2,B1 and B2 were 5.1%,6.1%,43.8% and 50%,respectively,while the incidences of LBWI were 4.4%,6.1%,43.8% and 41.7%,respectively.Multiple comparison shows statistically significant difference between groups(A1 vs B1,A1 vs B2,A2 vs B1,A2 vs B2,P < 0.001),there was no significant difference between both A1 vs A2 and B1 vs B2.The multivariate logistic regression analysis shows that the influence of Large ?BP on preterm and LBWI disappeared after the correction of HDP.Addition,we found HDP,premature rupture of memberanes(PROM)and inflammation,history of abortion,type of work were factors which influences the incidence of preterm while HDP and preterm influences LBWI.Conclusion The ?BP before and after pregnancy is not the risk factor of preterm and LBWI.HDP is an independent risk factor which effect the incidence of preterm and LBWI.
