Application Of Carpentier-Edwards Physio Ring And Classic Ring In The Treatmen Of Mitral Valve Moderately-severe Regurgitation In Recent Curative Effect Observation

Objective:By explore Carpentier-Edwards physio ring and classic ring in the treatment of mitral valve moderately-severe regurgitation in recent curative effect observation,summarize the clinical experience,provide new way to reduce the incidence of mitral valve moderately-severe regurgitation.Method:The study select 68 patients of mitral regurgitation from the first affiliated hospital of kunming medical university in September 2015-March 2017 as the research object,all of them have been the operation of mitral valvuloplasty,to analyze retrospectively the clinical data of them.36 of them use the Carpentier-Edwards physio ring as group 1(n = 36),32 of them use classic ring as group 2(n = 32).The effects of the two groups were evaluated and contrasted by echocardiography after operation.Results:(1)Surgical effect.The patients in both groups were successfully operated,and there were no complications and death during perioperation.NYHA was significantly improve after operation,.the level ? of Carpentier-Edwards physio ring was 31,and level ? was 5.There were 29 cases of classic ring is level ? and 3 cases of level ?.(2)Before operation,there was no significant difference between the two groups of LVEDD,LVESD,LVEF,LAD(P>0.05),but compared with pre-operation,the two groups of patients had a decrease of 1 week after surgery,half a year after surgery and lyear after surgery,the two groups of patients of LVEF had an ascent of 1 week after surgery,half a year after surgery and lyear after surgery,moreover,the group of Carpentier-Edwards physio ring was better than that of classic ring,and the comparison has statistically significant.The comparison of LVEDD has statistically significant after 1 week after surgery,half a year after surgery and 1 year after surgery(t=2.931,P<0.05;t=3.123,P<0.05;t=4.436,P<0.05);the comparison of LVESD has statistically significant 1 week after surgery,half a year after surgery and 1 year after surgery(t=3.269,P<0.05;t=4.383,P<0.05;t=5.279,P<0.05);the comparison of LVEF has statistically significant 1 week after surgery,half a year after surgery and 1 year after surgery(t=2.322,P<0.05;t=2.129,P<0.05;t=2.702,P<0.05);the comparison of LAD group has statistically significant 1 week after surgery,half a year after surgery and 1 year after surgery(t=2.311,P<0.05;t=4.379,P<0.05;t=3.597,P<0.05).(3)Follow up for 12 months through review,WeChat,letter,etc.there was no death.5 cases of residual mild reflux(2 cases of classic ring,3 cases of Carpentier-Edwards physio ring).In terms of NYHA classification,there were 33 patients in the Carpentier-Edwards physio ring group in grade ? and 3 in grade ?.The ? level of classic ring group is 30,and ? is 2.Conclusion:The treatment of severe MR with Carpentier-Edwards physio ring and classic ring can achieve remarkable results,Carpentier-Edwards physio ring has more consistent with the anatomy of the annulus,more conducive to improve left ventricular function,and more effective to avoid reflux recurrence and higher clinical value than classic ring.
