Report Of The Reconstruction Of Ulnar Coronoid Process And Literature Review

Purpose:Review our case of ulna coronary tu concrete method of reconstructive surgery and postoperative treatment,and review the related literature at home and abroad were reviewed,and clearly different ulna coronary reconstruction methods impact on clinical efficacy,to guide clinically appropriate reconstruction method is used to obtain better clinical curative effect.Methods:Retrospective analysis in our case of application of ilium as material and rebuild the ulna surgical methods and postoperative patients of the coronary bone graft healing and elbow joint function recovery,and the outburst of the ulna coronary reconstruction at home and abroad literature review,from coronary condyle fracture and elbow joint stability characteristics,coronary reconstruction of the indications,surgical approach and reconstruction of materials,fixation methods,postoperative care and surgical curative effect and so on a systemic research,analysis of different kinds of ulna coronary reconstruction methods impact on clinical efficacy.In this paper,the main points and points of attention in the reconstruction of coronal processes are clarified and the clinical curative effect is obtained.Results:Cases in this paper using dressing have a ilium as graft to replace the original coronary process of the ulna coronary reconstruction,postoperative follow-up function recovery in patients with elbow flexion and rotation is good,the operation has good curative effect.The author,through the literature found that due to coronary reconstruction indications of fracture is relatively narrow,most clinical ulna coronary tu treated by conservative treatment or open reduction and internal fixation,so the coronoid process reconstruction surgery in clinic does not see more,ulna are not found in the literature of the coronary reconstruction of large sample data,mostly sporadic cases have been reported,so the author for nearly 20 years of ulna coronary cases of reconstruction from reconstruction approaches to collect,reconstruction of materials,method of fixed,postoperative nursing and postoperative curative effect comparison,It was found that the choice of surgical approach was generally based on the combination of other fractures of the elbow joint and the injury of the surrounding soft tissue,as well as the previous surgical approach,reconstruction materials and fixation methods.In the reconstruction on the choice of materials,the author did not see through the literature standard but generally with radial capitulum,ulna olecranon tu,ilium for commonly used material,in this case,the ilium as a reconstruction of materials,and finely trimmed to achieve good matching and elbow;Piece of bone graft fixation with hollow screw,wire,gram,bone needle plate method,such as literature shows the hollow screw fixation for the mainstream,this case USES two hollow screw fixed iliac bone graft block from different angles,increase the stability of block bone graft;Coronary tu postoperative general routine use of plywood,gypsum or external fixator elbow braking,in this case,the use of hinged outside the frame fixed elbow,advantage is that can early line of elbow joint function rehabilitation exercise;The matching of reconstruction material and elbow joint and the time of elbow joint braking affect the short-term operation effect.Due to the ulna coronary reconstruction technique in the clinical application of time is not long,so the long-term clinical effect there is no adequate clinical trial results and statistical data support,still need scholars further follow-up study on the problem.Conclusions:1.The key of the ulnar coronal process is to carefully repair the reconstruction material to achieve a good match with the elbow joint,to maintain the concentric circles of the humerus joint.2.It is of great significance to maintain the stability of the elbow joint by probing and repairing the soft tissue damage during the operation.3.It is suggested to use hinged external fixator to fix the elbow joint after surgery,so that the elbow function can be recovered as soon as possible,which is beneficial to the rehabilitation of elbow joint function.
