Objective:Thyroid hormone(TH)is an important hormone that can regulate many organs and activities in the body.The heart is the direct target organ.Heart disease always associate with thyroid dysfunction.There are a number of studies have confirmed the molecular mechanism and clinical links of thyroid hormone and cardiac function.So how to use the thyroid hormone levels to assess the cardiac function,progress and prognosis,as well to prevent and treat heart failure(HF)has become the focus of research.This article intends to explore:1.The relationship between thyroid hormone levels and age in elderly patients with.2.The differences of thyroid hormone levels between chronic heart failure(CHF)patients and patients without CHF,the differences thyroid hormone levels between different cardiac functional grading.3.Similarities and differences of cardiac function grading between different thyroid hormone levels.To further assess the relationship of cardiac function and thyroid function in elderly patients,find out the more effective indicators to assess cardiac function and clinical prognosis of patients with chronic heart failure.Methods:A total of 611 elderly patients aged over 60 in Hebei general Hospital from February 2010 to March 2017 were selected,diagnosed as coronary atherosclerotic heart disease,stable angina,unstable angina,hypertension,rheumatic heart disease,hypertrophic cardiomyopathy,dilated cardiomyopathy,chest pain,valvular heart disease,heart failure.There were 339 males and 272 females.There were 305 non-heart failure patients,160 males and 145 females.306 patients with heart failure,179 males and 127 females.The basic information of these patients was recorded,and serum albumin(ALB),triglycerides(TG),fasting blood glucose(FBG),serum total triiodothyronine(TT3),total thyroxine(TT4),thyrotropin(TSH)and left ventricular ejection fraction(LVEF)were collected for analysis.SPSS 21.0 statistical software used for statistical processing.Results:1.The correlation between thyroid hormone and age levels The the level of TT3 were negative correlated with ages in all,HF group and NHF group(P<0.01).The the level of TT4 were negative correlated with ages in HF group(P<0.01).The TSH and age had no significant correlation in HF group,NHF group and in all.2.Comparison of thyroid hormones and general data between group HF and group NHF Compared with the NHF group,the content of TT3 in HF group were significantly lower(P<0.01),the content of TT4 was higher(P<0.05),the TSH were higher but no significantly different(P>0.05).Compared with the NHF group,the ages in HF group were significantly lower(P<0.05).Compared with the NHF group,the P,R,Hospitalization time in HF group were higher(P<0.01).Compared with the NHF group,the SBP,LVEF were significantly lower(P<0.01).3.Comparison of Thyroid Hormones in Different Cardiac Function Groups The level of TT3 in the NYHA Ⅱgroup,NYHA Ⅲ grup and NYHA Ⅳgroup was diffrenent(P<0.05),and it showed a gradually decreasing trend.There was a significant difference in diastolic blood pressure and respiration between the three groups(P>0.05),and it showed a gradual upward trend,;There was a significant difference in triglyceride and albumin between the three groups(P<0.05),and showed a gradually decreasing trend.4.Comparison of various indicators after grouping based on thyroid hormone levels According to the method of quartile,TT3 was divided into Q31,Q32,Q33,Q34 group from low to high.The number of NYHAⅡin Q31 group was lower than that in Q32,Q33,Q34 group(P<0.05).According to the quartile method,TSH was divided into Q1,Q2,Q3,Q4 from low to high,the number of NYHAⅢin Q4 group was higher than that in Q2 and Q3group(P<0.05),the number of NYHAⅣinQ4 group was higher than that in Q1,Q2 and Q3 group(P<0.05).Conclusions:1.The TT3 decreases with age in elderly patients.2.In elderly patients,TT3 in heart failure patients is lower than that in patients without heart failure and gradually decreases with the deterioration of cardiac function.It can be used as an objective index to evaluate the progression of patients with heart failure.3.When the heart function is not so bad,just as NYHA Ⅱ or Ⅲ,the body compensatory to increase TSH,if cardiac function is poor,just as NYHA Ⅳ,the compensatory function is poor,TSH does not increase or even decline,it can be used to monitor the progress of cardiac function as objective indicators.
