Objective:The research is a retrospective analysis of a series of clinical factors of the patients,such as the basic situation and the history of disease,who have been admitted to the hospital for subarachnoid hemorrhage,and then have the operation of the aneurysm clipping,to find out the risk factors of delayed intracranial hemorrhage after operation and looking for the way to reduce the occurrence and improve the prognosis of the patients.Methods:The research selects 545 cases who were in the Department of Neurosurgery in No.1 Hospital Branch of Jilin University from January2014 to January 2018.Collect the patient's medical history,include age,sex,history of smoking and drinking,hypertension,diabetes,cerebrovascular disease,GCS score,Hunt-Hess grade,coagulation routine,blood lipid at the time of admission,and the location,size,quantity and the number of ruptures of the aneurysm,modified Fisher grade which come from Imaging data of CT and CTA before the operation.They had the aneurysm clipping of unilateral or bilateral pterional within 72 hours after diagnosis of aneurysm.The operation and post operation management are carried out according to the unified standard,and make a tatistical analysis of related factors by means of single factor and multiple factors.Results:1.Delayed cerebral hemorrhage without aneurysm rupture was considered as a grouping variableto divide the patients into two groups,and analyze with?~2 test.The result is GCS score,modified Fisher grade,Hunt-Hess grade,hypertension,diabetes,hyperlipidemia at admission have a significant correlation with delayed cerebral hemorrhage(P<0.05),and age,sex,history of smoking and drinking,cerebrovascular disease,coagulation abnormality,and the location,size,quantity and the number of ruptures of the aneurysm have no significant correlation.2.The results of multiple factor Logistic regression analysis is that diabetes,hyperlipidemia,Hunt-Hess grade and GCS score at admission have a significant correlation with delayed cerebral hemorrhage after the operation of the aneurysm clipping(P<0.05),and hypertension and modified Fisher grade have no significant correlation(P>0.05).Conclusion:1.Diabetes,hyperlipidemia,Hunt-Hess grade and GCS score at admission have a significant correlation with delayed cerebral hemorrhage after the operation of the aneurysm clipping.2.Age,sex,history of smoking and drinking,cerebrovascular disease,coagulation abnormality,hypertension,modified Fisher grade,and the location,size,quantity and the number of ruptures of the aneurysm have no significant correlation with delayed cerebral hemorrhage after the operation of the aneurysm clipping.
