Objective:To discuss the effect of the deltoid pectoralis major muscle approach and the anterolateral acromial approach in the treatment of proximal humeral fractures in the elderly.Methods:We randomly selected 82 patients of proximal humeral fractures in the elderly that be admitted to the department of orthopedics from Aug.2015 to Nov.2017 in the first houspital of Jilin University.As the subject of this investigation,82 patients were randomly divided into experimental group(41 cases)and control group(41 cases)according to the number.Patients in the experimental group adopted the anterolateral acromial approach,and the control group used the deltoid pectoralis major muscle approach.4-6 months of follow up after treatment,intraoperative bleeding volume,hospitalization time,operation time,visual analogue score(VAS)of preopration and postopration,healing time of fracture,complication rate,and the function recovery rate of shoulder joint were compared and analyzed of the two groups.Results:The intraoperative bleeding volume in the experimental group was significantly less than the control group(P < 0.05).The hospitalizationtime,operation time and the healing time of fracture in the experimental group were significantly shorter than that of the control group(P < 0.05).The VAS and the incidence of complications in the experimental group were significantly lower than that of the control group.The function recovery rate of shoulder joint in the experimental group was significantly higher than that of the control group.The differences between groups were statistically significant.Conclusions:The anterolateral acromial approach in treatment of senile proximal humeral fracture has good clinical effect,it can effectively reduce the amount of bleeding,shorten the operation time and hospitalization time,preoperative and postoperative pain of patients is relatively low,and reduce the incidence of complications,shoulder joint function better accepted by patients.It is worthy of promotion and application.
