Objectives:The objective of this thesis lies in that,for one thing,gaining the knowledge of the relationship between red blood cell distribution width in elderly patients with chronic heart failure and diabetes mellitus,for another,discussing the significance of red blood cell distribution width in anticipating prognosis of the elderly patients with chronic heart failure.Methods:Altogether collecting 245 cases of elderly patients(aged over 60,with hospitalization days from Sept,2015 to Apr,2016)from the “New Cadre” ward of the First Hospital of Jilin University.Among the 245 cases,163 are cases of CHF patients(combined with 51 cases of diabetes mellitus),leaving the rest 82 cases as the control group.Haematological index,electrocardiogram,and Cardiac Doppler ultrasound are measured and recorded the second day of hospitalization of the patients.Follow-ups(with an average time duration of 15.9 months,ranging from0.3 to 21 months)are carried on after their discharging from hospital,ended by readmission to hospital caused by HF,all-cause mortality,or expiration of 21 months.According to whether combined with diabetesmellitus,the cases are divided into three groups: the control group,the CHF group,and the CHF combined with Diabetes Mellitus group.Besides,according to the end point of follow-up,the cases are divided into two groups: event group and non-event group.Moreover,according to RDW value the quatiles are marked as Q1,Q2,Q3 and Q4.Results:1.Compared with the simple chronic heart failure group and diabetic roup,red blood cell distribution width,survival time,left ventricular myocardial mass index,fasting blood glucose,glycosylated hemoglobin there are differences.2.Incidence of diabetes,smoking and alcohol consumption,combined left ventricular hypertrophy,hospitalization days,Log[NT-pro BNP],RDW,PDW,Cr,BUN,etc.in the event group were higher than those in the non-event group.LVEF,survival time,TG,HGB is significantly lower than the non-event group;3.There were significant differences in the incidence of Q1-Q4 between the four groups,prevalence of diabetes,left ventricular hypertrophy,hospitalization days,survival time,LVEF,and Log[NT-pro BNP].The incidence of events with higher RDW values,prevalence of diabetes,left ventricular hypertrophy,hospitalization days,Log [NT-pro BNP] were significantly higher than those of the lower group,and survival time and LVEF were significantly lower than those of thelower RDW values group.4.From the survival function curve,it can be seen that the survival rate of Q1 group is greater than Q2 group,which is greater than Q3 group and Q4 group.With the increase of RDW values,the survival time gradually decreased.With the extension of time,the survival rate of Q2,Q3 and Q4 groups declined rapidly.5.As RDW values increase,Log[NT-proBNP] also increases,and RDW levels is positively correlated with Log[NT-proBNP].Conclusion:1.There is a correlation between RDW and diabetes mellitus in elderly patients with chronic heart failure.2.RDW can be used as an effective index to assess the severity and prognosis of elderly patients with chronic heart failure.3.RDW is an independent influencing factor of adverse events in elderly patients with chronic heart failure.
