Objective: To discuss the risk factors of the recurrence of endometrial cancer,the values of age and lymphovascular space invasion in the prognostic factors of endometrial cancer were analyzed and discussed.Methods: One hundred and eight patients with endometrial cancer(those who with new adjuvant therapy and who without prior staging operation were excluded)in our hospital from January 2008 to December 2012 were analyzed retrospectively.The median follow-up time was 43 months(range from 3 to 96 months).The survival curves were then estimated by the Kaplan-Meier method.The hazard ratios(HRs)and 95% confidence intervals(Cis)for PFS and OS were calculated by the way of Cox proportional hazards models.The values of age,BMI,basic diseases,depth of myometrium invasion,pathological type,histological grade,lymph node metastasis,lymphovascular space invasion,postoperative treatment on recurrence of endometrial cancer were studied.P<0.05 means the differences has statistical significance.Results: 1.A total of 108 cases of endometrial cancer were included in this study.The results of Univariate logistic regression analyses indicated that age、lymph node metastasis、myometrial invasion、lymphovascular space invasion and adjuvant therapy were the factors which had statistical significance,and then the factors were analyzed by the way of Cox proportional hazards models.2.Multivariate logistic regression models were used to adjust for the influence of factors.Only lymph node metastasis(HR = 3.77,95% CI:(1.38~10.28,P = 0.01),deep myometrial invasion(HR = 3.69,95% CI: 1.37~9.92,P = 0.01),lymphovascular space invasion(HR = 3.69,95% CI: 1.45 9.40,P = 0.006)were the independent prognostic factors of endometrial cancer.3.Age older than 60(HR = 0.87,95% CI: 0.33~2.29,P = 0.77)was not the independent prognostic factor.Conclusions: 1.Deep myometrial invasion(DMI),lymph node metastasis(LNM),lymphovascular space invasion(LVSI)were the independent risk factors of the recurrence of endometrial carcinoma.2.Age older than 60 was not the independent prognostic risk factor.There was no other risk factor in the clinical practice,but age older than 60,what we should do was postoperative observation.3.LVSI was an important independent prognostic factor for recurrence(local recurrence and distant recurrence)and survival,regardless of the depth of myometrial invasion and lymph node status,lymphovascular space invasion seems to be a better predictor.
