Changes And Correlation Analysis Of Serum TLR9,IL-8 And S100A8 Levels In Patients With Type 2 Diabetic Foot Ulcers

[Objective]The research was aimed at investigating the role of TLR9,IL-8 and S100A8 in the pathogenesis and development of diabetic foot ulcers(DFU)by observing the changes of serum levels of TLR9,interleukin-8(IL-8)and calcium binding protein(S100A8(calcium-binding protein S100A8)in patients with diabetic foot ulcers.[Methods]90 cases of medical examination in Department of Endocrinology,Kunming general hospital,Kunming General Hospital of the PLA from January 2016 to October 2017,among them,there were 30 cases of type 2 diabetes without foot ulcers(group DM),30 patients with diabetic foot ulcers(group DFU)and 30 normal control group without diabetes(N Group C).The general clinical data(sex,age,course of disease,BMI,SBP,DBP)of the three groups of subjects were collected and their biochemical indexes(FPG,HbA1c,TC,TG,LDL-C,HDL-C)were detected.The concentrations of TLR9,IL-8 and S100A8 in the serum of the three groups were detected by ELISA.The correlations between TLR9 and other factors were analyzed,and the influencing factors of diabetic foot were analyzed by logistic regression.Each index of the three groups was tested by homogeneity of variance.The single factor analysis of variance was used in the comparison of each index of the same variance,and the difference of the indexes among the three groups was compared by Bonferroni test.Welch's analysis of variance and Games-Howell test were used to compare the differences among the three groups.?2 test was used to compare the classified counting data between groups.Pearson correlation was used to analyze the correlation between TLR 9 and other indicators.Logistic regression analysis and risk analysis were performed on the factors of diabetic foot.[Results](1)There was no significant difference in gender,age,BMI and DBP between the three groups in group DFU,group DM and group NC(P>0.05).(2)There was no statistical difference in TC,TG and HDL-C(P>0.05)in group DFU and group DM(P>0.05),and there were statistically significant differences in the course of disease,HbAlc,LDL-C,SBP,FPG,TLR9,IL-8,and S100A8(P<0.05).(3)Compared with group NC,there were significant differences in SBP,FPG,HbAlc,TC,TG,LDL-C,HDL-C,TLR9,IL-8,and S100A8 between group DFU and group NC(P<0.05).(4)There was no statistical difference in TLR9 and LDL-C between group DM and group NC(P>0.05),and there were statistical differences in TC,TG SBP,FPG,HbAlc,HDL-C,IL-8,and S100A8(P<0.05).(5)There was a significant positive correlation between TLR9 and SBP,FPG,HbA1c,IL8 and S100A8(r=0.231,0.381,0.487,0.474,0.519,P<0.05),and there was a significant negative correlation between TLR9 and HDL(r=-0.275,P<0.05).(6)Logistic regression analysis of diabetic foot ulcers and other factors showed that SBP,HbAlc,LDL and TLR9 were statistically significant in the model(P<0.05)and were risk factor affecting the occurrence of diabetic foot ulcers.Once the amount of these factors increased a unit,the risk of diabetic foot ulcers increased to 1.024 times(1.001-1.047),1.748 times(1.198-2.550),2.632 times(1.258-5.508),1.868 times(1.266-2.756),respectively.[Conclusions](1)Serum levels of TLR9,IL-8 and S100A8 in patients with diabetec foot ulcers increased significantly.(2)The level of serum TLR9 in diabetic foot ulcers was positively correlated with SBP,FPG,HbA1c,IL8 and S100A8,and negatively correlated with HDL.(3)SBP,HbAlc,LDL and TLR9 are risk factors for diabetic foot ulcers.
