Objective:1.To explore the role of ApoE gene polymorphism in the pathogenesis of type 2 diabetic nephropathy.2.to explore the role of SLCO1B1 gene polymorphism in the pathogenesis of type 2 diabetic nephropathy.3.To explore the role of lipid metabolism in the pathogenesis of type 2 diabetic nephropathy.Methods:In this study,a total of 166 subjects were selected,including 100 patients with type 2 diabetes.According to urinary albumin/creatinine ratio,type 2 diabetes group was divided into diabetic nephropathy group(DN+)and non diabetic nephropathy group(DN-).There were 60 cases in diabetic nephropathy group and 40 cases in non diabetic nephropathy group.There were 60 cases in the control group.We collected the clinical data of each group,including age,sex,height,weight,BMI,TC,TG,LDL-C,HDL-C and microalbuminuria.Finally,we compared the genotype and allele frequencies of ApoE and SLCOIBI and various blood lipid indicators in each group,and calculated whether there was significant difference in their distribution.Results:1.In the population of Yunnan,three kinds of alleles(epsilon 2,epsilon 3 and epsilon 4)and six genotypes(?2/?2,?3/?3,?4/?4,s2/?3,?2/?4,?3/s4)were detected in ApoE gene.Two alleles(A and G)and three genotypes(AA,AG and GG)were detected in SLCOIBI 388A>G site.Two alleles(T and C)and three genotypes(TT,TC and CC)were detected in SLCOIB1 521T>C sites.The two sites constitute three haploid types(SLCO1B1*15,SLCOIBI*1a and SLCO1B1*1b).2.Compared with the DN(-)group,the high density lipoprotein cholesterol(HDL-C)in the DN(+)group decreased significantly,and the difference was statistically significant(P<0.05).3.Compared with the control group and the DN(-)group,the frequency of ApoE?2/?2+?2/?3 genotype in the DN(+)group increased significantly,and the difference was statistically significant(P<0.05).4.Compared with the control group and the DN(-)group,the frequency of ApoE ? 2 allele in the DN(+)group increased significantly,and the difference was statistically significant(p<0.01).5.The triglyceride level in type 2 diabetic patients was gradually decreased in the ApoEs2/?2+?2/?3??3/4?+?4/4?s3/s3,and the difference was statistically significant(P<0.05).6.There was no significant difference in genotype and allele frequency of SLCO1B1 388A>G and 521T>C and haploid frequency of SLCO1B1 gene in the control group,DN(-)group and DN(+)group.7.Logistic regression analysis showed that high density lipoprotein cholesterol decreased,ApoE s2/s2+s2/s3 genotype and ApoE ?2 allele were risk factors for type 2 diabetic nephropathy.Conclusion:1.The polymorphism of ApoE gene is associated with the occurrence of type 2 diabetic nephropathy,ApoE ?2/?2+?2/s3 genotype and ApoE ?2 allele are risk factors for type 2 diabetic nephropathy.2.There was no significant correlation between SLCO1B1 388A>G and 521T>C gene polymorphisms with type 2 diabetes and type 2 diabetic nephropathy.3.The decrease of high-density lipoprotein cholesterol is a risk factor for type 2 diabetic nephropathy.4.The polymorphism of ApoE gene is associated with triglyceride level in type 2 diabetic patients.The level of triglyceride is higher in patients with genotype ?2/?2+?2?3.
