Objective: To evaluate the value analysis of pulmonary CT in predicting tuberculous hemoptysis and its guiding effect on interventional treatment of tuberculous hemoptysis.Methods: The materials about 60 patients with tuberculosis,from January 2017 to January 2018 in Changsha Central Hospital which are collected,who divided into 30 hemoptysis group and 30 non-hemoptysis group are analyzed retrospectively.The CT findings of hemoptysis patients and non-hemoptysis patients were compared to research the pulmonary lesions mostly associated with hemoptysis,and the distribution of responsible blood vessels found in the same patient with DSA was compared.SPSS 18.0 software is used for statistical analysis to see if the pulmonary CT findings could predict the responsible blood vessels for hemoptysis.Results:(1)There is no significant difference in age,sex ratio,preoperative sputum smear and sputum culture and tuberculosis symptoms in patients with hemoptysis group and non-hemoptysis group(P > 0.05).(2)tuberculosis hemoptysis patients in tuberculous cavity,tuberculous cavity with gas-fluid level,tuberculous cavity with secondary aspergilloma,ground-glass opacity,bronchiectasis,and pleural thickening percentage of CT performance comparison of hemoptysis high groups(table 2),there are statistically significant differences(P < 0.005);There are 141 blood vessels relating with hemoptysis to be found under DSA in the 30 hemoptysis patients.The proportion of the posterior intercostal artery was the highest,with 30.50%.(4)application of tuberculous cavity,bronchiectasis,ground-glass opacity,pleural thickening of the four kinds of lung CT lesions in 30 patients with hemoptysis group corresponding lung lesions appear responsible blood vessels(responsible blood vessels is found under DSA as the gold standard)to forecast;the result shows: the application of ground-glass opacity shadow tuberculous hemoptysis to predict the sensitivity,specificity and accuracy of the highest,85.71%,96.25% and 91.33% respectively.Conclusion:1.There are obvious differences between tuberculosis hemoptysis and tuberculosis non hemoptysis on the type of lesion of CT.2.The accuracy of predicting the responsible artery of tuberculosis hemoptysis is different in different lesions of pulmonary CT.3.According to the type of lesion on pulmonary CT before operation,the distribution position is helpful to predict the distribution area of responsible artery for hemoptysis in advance before interventional therapy.To a certain extent,the operation time is saved,the leakage of thrombus is reduced,and the success rate and curative effect of embolization hemostasis are improved.
