Objective To observe the incidence and symptoms of dry eye in patients with diabetic retinopathy(DR),and study the relationship between diabetic retinopathy and dry eye as well as discuss the possible causes of dry eye in DR patient,which would provide theory gist for early intervention treatment of dry eye in DR patient.Methods Patients diagnosed with type 2 diabetes(fasting blood glucose>7.0mmo1/L,postprandial blood glucose after 2h?11.1mol/1)were recruited from January 1st,2016 to December 1st,2016 in Department Endocrinology,General Hospital of Guangzhou Military Command Department Endocrinology.According to the results of fundus fluorescein angiograwhich phy,forty patients diagnosed with with proliferative diabetic retinopathy(PDR),was group A,and 32 patients diagnosed with non-proliferative diabetic retinopathy(NPDR),which was group B,as well as 36 healthy subjects(group C)were included respectively.Evaluating the symptoms of dry eye in these groups by means of OSDI questionnaires,Schirmer I test(SIT),non-invasive breakup time(NIBUT),lipid layer analysis were presented in different groups.Sodium dodecyl sulfate-polyacrylamide gelelectrophoresis(SDS-PAGE)was used to analysis the level of tear protein.ANOVA,Bonferroni and Logistic were used in statistical analysis.Results(1)Diabetic patient tend to get dry eye.Among the 72 patients with type 2 diabetes mellitus,the percentage of dry eye was 61.11%,while PDR was 70.00%and NPDR was 50.00%respectively.(2)Group A and B were positively associated with the incidence of dry eye,with OR was 17.00 in group B and 57.29 in group A respectively.(3)As the degree of DR worsened,the OSDI questionnaire score increased significant,while the result of NIBUT,SIt,the quantity of goblet cell decreased in group A and B when compared to group C.? The score of OSDI questionnaire score in group A,B,C were(64.32± 1.00),(28.45± 1.10)and(13.54±0.90)respectively(P<0.001).? The seconds of NIBUT in group A,B,C were(5.90±2.28)s,(7.58 ±2.15)s and(11.55 ±2.25)s respectively(P<0.001).? The result score of SIt were(7.75 ±2.03)mm,(8.75±1.98)mm and(10.78±2.21)mm in group A,B and C respectively(P<0.001).?The number of goblet cells in group A,B,C were(59.00±5.18),(67.5±3.93)and(80.94±8.87)respectively(P<0.001).?Compared to group B,PDR group A had a significant reduction inlysozyme,lactoferrin(P<0.05)and tear specific prealbumin concentration(P<0.01).There is no differences in lysozyme,lactoferrin and tear specific prealbumin between NPDR group B and group Ccontrol group(P>0.05).Conclusion 1.Patients with diabetes mellitus were susceptible to dry eye than healthy people.2.The incidence of dry eye was related to the severity degree of DR positively.Therefore we should take attention to think highly of the relationship between the dry eye and diabetic retinopathy,and intervention treatment of ocular surface disorders should be taken at early stage.
