Backgroud:Pulmonary arterial hypertension(PAH)is a cardiovascular disease characterized by a progressive increase in pulmonary arterial pressure due to primary arterial proliferative lesions.The pathological features are mainly caused by pulmonary vascular remodeling and elevating pulmonary vascular resistance(PVR),resulting in right ventricular dysfunction and right heart failure.Objective:To investigate the relationship between YKL-40 and miR-30a-5p and the severity and prognosis of patients with pulmonary hypertension.Methods:Plasma YKL-40 and miR-30a-5p levels were measured in 71 patients with PAH from 2015 to 2018 in Guangdong General Hospital and 35 controls.All the PAH patients underwent YKL-40 and miR30a-5p?Echocardiography(UCG)and right heart catheterization(RHC)measurement.Compare the YKL-40 and miR30a-5p levels between two groups and the relevant between YKL-40 and miR30a-5p and the clinical indicators by.All the patients were followed up for 6-30 months.Clinical deterioration or death was recorded.The ability of YKL-40 and miR-30a-5p to predict the prognosis of patients with PAH was evaluated by ROC curve and Kaplan-Meier,,and COX regression analysis model.Results:The level of YKL-40 and miR30a-5p of all the patients was higher than the controls.Correlation analysis suggested that YKL-40 was negatively correlated with RVCO?RVCI,miR-30a-5p was positively correlated with mPAP and PVR.ROC curve analysis suggested that YKL-40 has higher sensitivity and specificity for predicting clinical deterioration;miR30a-5p has higher sensitivity and specificity for predicting mortality.Compared to patients with miR-30a-5p Below the ROC-derived cut-off point(12.225),patients with miR-30a-5p? 12.225 show worse prognosis.Conclusion:YKL-40 and miR30a-5p levels were significantly elevated in patients with PAH complicated with severe cardiac insufficiency,and were associated with partial hemodynamic parameters of PAH.YKL-40 and miR30a-5p might be used as indicators for judging the severity and prognosis of patients with pulmonary hypertension.
