Objective: To explore the efficacy and safety of autologous blood collection therapy for patients with high volume refractory acute heart failure.Methods:24 patients with Refractory Heart Failure with High Capacity who were in Xiangtan city Central Hospital from September 2015 to March 2018 were included as cases of this research.According to the randomized control principle, all of them were divided into two groups: blood collection therapy group and conventional treatment group, each of which included 12 patients.Conventional anti-heart failure treatment combined with autogenous blood collection was adopted in the blood collection treatment group, and conventional anti-heart failure treatment was adopted in the conventional treatment group. In this research, respiratory difficulty score, blood oxygen saturation, arterial oxygen partial pressure and NT-pro BNP were the index of the Acute Heart Failure alleviation, central venous pressure(CVP),inferior vena cava diameter(IVC),pulmonary B line were the evaluation index of the volume load, and heart rate, hemoglobin(Hb),serum creatinine, uric acid uric acid Sodium ion(Na+)potassium ion(K+)were a safety evaluation index,to explore the efficacy and safety of autologous blood collection therapy for patients with high volume refractory acute heart failure. Results: During the treatment,no malignant cardiovascular events occurred in the blood collection treatment group and the conventional treatment group. In pursuance of the treatment group compared to the conventional treatment group, blood oxygen saturation, dyspnea score before treatment, there was no statistically significant difference(p > 0.05),after treatment, blood treatment group blood oxygen saturation and respiratory difficulty degree of score improved significantly compared with the conventional, the difference was statistically significant(p < 0.05);There was no statistically significant difference in the comparison of pretreatment arterial oxygen partial pressure(p>0.05).After treatment, the increase in the blood collection treatment group was significantly higher than that in the conventional treatment group, and the difference was statistically significant(p <0.05).Before treatment, there was no statistically significant difference in nt-probnp values between the two groups(p>0.05).After treatment, the decrease rate of blood sampling treatment group was significantly higher than that of the conventional treatment group, and the difference was statistically significant(p <0.05).The differences in heart rate, hemoglobin, serum creatinine, serum urea nitrogen, uric acid,Na+ and K+ between the two groups before and after treatment were not statistically significant(p>0.05).Conclusions:1.Compared with conventional anti-heart failure treatment, combined with autologous blood collection therapy can more effectively reduce the capacity load of patients with high capacity refractory acute heart failure and alleviate the condition of heart failure.2.Autologous blood collection is relatively safe for patients with high capacity refractory heart failure.
