To explore the clinical value of chitinase-3-like protein-1,hypersensitive c-reactive protein and homocysteine expression in assessing the disease status of patients with coronary heart disease and predicting unstable plaques.Further clarify the clinical significance of YKL-40 as a predictor of vulnerable plaque,risk stratification,and long-term prognosis in CHD patients.This study selected 193 cases of hospitalized patients with cardiovascular internal medicine in July 2016 to April 2017 in northern hebei institute of the first affiliated hospital.According to the clinical symptoms and the CAG results,the research objects were divided into CHD group(n=145)and the control group(n=48).Patients in CHD group were examined by clinical symptoms laboratory,and CAG showed that the stability of coronary artery lesions and plaques were divide into acute myocardial infarction(AMI)(n=48),unstable angina(UA)(n=49),Stable angina(SA)(n=48).The control group was 48 patients with no obvious stenosis of coronary artery.According to the CAG results,the patients with acute coronary syndrome were divided into single lesion group(n=32),bilateral lesion group(n=31)and multiple lesion groups(n=34).Plasma level of YKL-40,hs-CRP and Hcy measured by enzyme-linked immunosorbent Assay method.Plasma level of total cholesterol,triglycerides and low density lipoprotein cholesterol measured by Hitachi 7170 a full-automatic biochemical analyzer.Using Ambrose evaluate the nature of the classification of plaque in patients with coronary heart disease.Plasma YKL-40,hs-CRP and Hcy levels in the ACS group were all significantly higher than those in the SA group and the control group(P<0.05).Plasma YKL-40,hs-CRP and Hcy levels in the AMI group were all significantly higher than those in the UA group,SA group and the control group(P<0.05).In the ACS group,the plasma level of YKL-40 in the multi-branch lesions group was significantly higher than that in the double-branch lesions group and the single-branch lesions group(P<0.05).There was no statistically significant difference in hs-CRP and Hcy levels between the multiple lesion group,the double-branch lesion group and the single lesion group(P>0.05).The expression levels of YKL-40,hs-CRP and Hcy were related to the degree of disease and the stability of VP in CHD patients.The combined measurement of the three levels is of great clinical significance to the disease assessment of CHD and the early identification of unstable plaques.The expression level of YKL-40 was related to the extent of coronary lesions and stenosis in ACS patients.It is of great clinical value to predict the stability of atherosclerotic plaque and the occurrence of ACS events in CHD patients by detecting YKL-40 level.
