Study On The Relationship Between The Level Of Tsh And The Incidence Of Permanent Hypothyroidism In Patients With Graves Disease After ¹³¹I Treatment

Objective:¹³¹I has become the most commonly used treatment for Graves’ s disease.Hypothyroidism is a natural outcome of most Graves disease patients treated with radioactive iodine and is an inevitable complication after ¹³¹I treatment of Graves disease.¹³¹I is the most safe,low cost,high curative effect,low recurrence rate and wide application method in the treatment of GD at present,but it is not accepted by many patients because of the effect of hypothyroidism after treatment.To avoid and reduce permanent hypothyroidism under conditions that ensure a cure rate Generation is a key issue.Numerous studies have found that early hypothyroidism may require life-long thyroid hormone replacement therapy if it is not given in time and to become permanent hypothyroidism.In this study,we observe the incidence of permanent hypothyroidism,early hypothyroidism of GD used ¹³¹I treatment use Levothyroxine Sodium Tablets to make Thyroid Stimulating Hormone at different level.To provide the basis for clinical treatment of GD after ¹³¹I treatment of hypothyroidism.Method:105 patients with GD in the Department of endocrinology of2016.03-2016.11 First Affiliated Hospital of Hebei North University were selected for ¹³¹I treatment.All patients were followed up for a year.Based on the results of one year follow-up,cured and hypothyroidism were classified into effective treatment.One hundred and five GD patients ¹3131 I three months after treatment,seventy patients with diagnosed with hypothyroidism,were randomly divided into group A（thirty-five cases）:to give Jia heng replacement therapy,are keeping TSH normal level（0.27-4.2 mIU/L）;Group B（thirty-five cases）:the replacement therapy was given to make TSH maintain a Slightly higher than normal（4.2-10miu/L）.After one year of treatment,to observe the incidence of permanent hypothyroidism.Results:After ¹³¹I treatment in GD patients,17 cases returned to normal（16.2%）,70 cases with hypothyroidism（66.7%）,8 cases with recurrence（7.6%）,10 cases with no effect（9.5%）.The effective rate of ¹³¹I in treating GD was 80.6%.The effective rate of ¹³¹I treatment was 80.6%;TSH maintained at normal level（0.27-4.2 mIU/L）in patients with permanent hypothyroidism incidence was 88.6%,a little higher TSH maintained in the normal level（4.2-10 mIU/L）in patients with permanent hypothyroidism incidence was 65.7%,the difference was statistically significant（P<0.05）.Conclusion:After ¹³¹I treatment in GD patients,the incidence of permanent hypothyroidism could be reduced by giving Jia heng substitution therapy to keep TSH at a Slightly higher than normal.
