Objective:To explore the surgical method of tendinous mallet finger and to evaluate the clinical effect of S-shaped wire in treatment of tendinous mallet finger.Methods: Follow-up 59 cases of surgical tendinous mallet fingers from May 2012 to July 2016 at The Liuhe Hospital Affiliated to Medical College of Yangzhou University,including 35 males and 24 females,aged from 16 to 72 years old,at the average age of 41 years old ?There were 4 cases of thumbs 8 cases of the index finger,16 cases of the middle finger,19 cases of the ring finger,12 cases of the little finger.The injury causes included stab wounds in 16 cases,sprain in 20 cases,crush injur in 18 cases and fall in 5 cases.The treatment time after injure was between 1 hours to 60 days.The patients were divided into two groups according to the different surgical methods,there were 34 cases of the S-shaped wire group,and the number of cases in the anchor group was 25.The follow-up contents were mainly the general information of patients,surgical methods,preoperative and postoperative finger's distal interphalangeal joints active and passive activity,postoperative complications.Evaluation standard adopts Crawford system evaluation method.All data were analyzed by statistical analysis,using IBM SPSS 19 statistical software for statistical analysis,P < 0.05,indicating that the data was statistically significant.Results: Postoperative follow-up time is more than six months.After operation,59 cases were all followed up.Record and compare the data of related imaging,length of hospital stay,the duration of operation,hospitalization costs,postoperative finger's distal interphalangeal joints active and passive activity and postoperative complications.The average operation time of the anchor nail group was?47.24±5.49?minutes,and the average operation time of the S-shaped wire group was?47.85±4.75?minutes,P>0.05,and the two groups had no statistically significant differences in operative time.The average hospital stay of the anchor nail group was?7.60 rb 1.80?days,and the average hospitalization stay of the S-shaped wire was?7.44 dr 1.76?days,P>0.05,there was no statistical difference between the two groups.The average hospitalization expenses of the anchor nail group was?7451.32 ± 170.97?RMB,the average hospitalization expenses of the S-shaped wire group was?3373.21 dr 136.80?RMB,P<005,the two groups have statistical significance.The hospitalization expenses of the anchor nail group was significantly higher than that of S-shaped wire group.The postoperative curative effect was evaluated by Crawford.The postoperative functional recovery in the 34 cases of the S-shaped wire group was recorded,9 cases were excellent,21 cases were good,3 cases was acceptable,1 case was bad,The excellent and good rate was 93.33%.The postoperative functional recovery in the 25 cases of the anchor nail group was recorded,4cases were excellent,14 cases were good,5 cases was acceptable,2 cases was bad,The excellent and good rate was 72.00%.Statistical analysis showed that there was no significant difference in the functional evaluation between the two groups?P>0.05?,Therefore,we believe that there is no difference in the efficacy of the two surgical methods.The type of complication was inconsistent between the two groups.Complications of S-shaped wire group showed 2 cases of nail root injury,3 cases of tendon adhesion and 2 cases of wound infection.Complications of the anchor nail group included 2 cases of the line junction rejection reaction,1 case of nail root injury,2 cases of tendon adhesion and 1case of wound infection.Two cases had complications and no complications,and compared with P>0.05 by X2 test The results showed that there was no difference between the two methods in the complications.Conclusions: There was no significant difference in the recovery of the postoperative DIP function of the S-shaped wire group compared with the anchor nail group.The curative effect of S-shaped wire is positive,a novel,a economical and practical method,and worthy of spreading.
