Objective: To investigate the effect of ultrasound-guided transversus abdominis plane block(TAPB)for postoperative pain in elderly patients and to provide evidence for clinical application.METHODS: A total of 400 elderly patients who underwent surgery in our hospital from September 2016 to December 2017 were selected and divided into observation group and control group according to the different anesthetic methods.Ultrasound-guided transversus abdominis fascia block complex conventional vein analgesia and conventional intravenous analgesia were used.The postoperative analgesic effect and recovery were analyzed retrospectively.Observed and compared the patient's pain(visual analog scale VAS),vital signs(MAP,HR),adverse reactions.Results: The usage of propofol in the observation group and control group were(458.47±134.22)mg?(675.32±140.69)mg,and remfentanyl dosage were(906.78±107.89)?g?(1512.56±152.90)?g,respectively,comparison between the group represent P<0.05.The MAP and HR at the end of the surgery were compared,P>0.05;When the mask was removed,the MAP and HR groups in the observation group and the control group were compared at P<0.05;Comparison of operating room time between two groups of patients,observation group was lower than the contral by P<0.05.In the observation group,the pain at 2,6,and 12 hours after operation was lower than that in the control group(P<0.05).The pain was compared at 24 hours,P> 0.05.The two groups of average recovery time,adverse reactions compared,P<0.05.Conclusion: Ultrasound-guided TABP is superior for postoperative analgesia in the elderly.It has quick recovery,low pain,few adverse reactions,high safety.It can be clinically applied. |
