Objective:To study the value of haptoglobin and sCD163 in the differential diagnosis of tuberculous and malignant pleural effusion and its correlation with the inflammatory response of the body.Objects and methods:Patients with pleural effusion treated in Respiratory Medicine Department of The First Hospital of Jilin University from April 2017 to January 2018 were enrolled in this study,collecting and preconditioning the initial pleural effusion.A total of 78 patients were brought into this study after strict selection and exclusion criteria.According to strict grouping criteria,the patients were divided into two groups: tuberculous pleural effusion group(TPE,38 cases)and malignant pleural effusion group(MPE,40 cases).The general data of the patients were collected.The concentrations of haptoglobin and sCD163 in pleural effusion supernatant of the patients were detected by ELISA.Using SPSS18.0 statistical software analysed the differential concentration of haptoglobin and sCD163 expressed in TPE and MPE,and the correction of haptoglobin,sCD163 and the inflammatory response of the body,then received the conclusion.Result:1.The concentration of haptoglobin in tuberculous pleural effusion and malignant pleural effusion were 1374.94 ± 411.13 ug / ml and 505.84 ± 294.65 ug / ml,respectively,the difference was statistically significant(P <0.05);The concentration of sCD163 in tuberculous pleural effusion and malignant pleural effusion were 2102.58±611.59ng/ml and 1240.62±428.74ng/ml,respectively,the difference was statistically significant(P <0.05)2.The cut-off value of haptoglobin in pleural effusion for the differential diagnosis of TPE and MPE was 779.05ug/ml,with the corresponding sensitivity of 82.4% and specificity of 86.1%,respectively(P<0.01).The cut-off value of sCD163 in pleural effusion for the differential diagnosis of TPE and MPE was 1640.11ng/ml,with the corresponding sensitivity of 76.3% and specificity of 85%,respectively(P<0.01).The corresponding sensitivity and specificity were 90% and 87.5% when haptoglobin and sCD163 were combined used in the differential diagnosis of TPE and MPE,respectively.3.Haptoglobin was positively correlated with interleukin 1β(IL-1β),tumor necrosis factor α(TNF-α),C-reactive protein(CRP),e r y t h r o c y t e s e d i m e n t a t i o n r a t e(E S R)i n TPE group and MPE group(P<0.05);Haptoglobin was positively correlated with s CD163 in TPE group(r=0.3735 P=0.0209),but it was no correlation in MPE group(r=0.22 P=0.1684).There was no correlation between s CD163 and tumor necrosis factor α,C-reactive protein,e rythrocyte Sedimentation rate in the TPE and MPE group(P>0.05).sCD163 and interleukin 1β were weakly correlated in TPE group(r=0.49 P=0.0018),but there was no correlation in MPE group.(r=0.068 P=0.6767).Conclusion:1.Haptoglobin and sCD163 can be used as potential markers for the differential diagnosis of pleural effusion.2.The haptoglobin may reflect the intensity of inflammatory response of the body in tuberculous pleural effusion and malignant pleural effusion. |
