Objective:To study the clinicopathologic characteristics,prognosis,and survival analysis of papillary thyroid carcinoma in children and adolescents.Method:The study subjects were 94 children and adolescents all treated by thyroid surgery due to papillary thyroid carcinoma,from Jan.2000 to Jan.2013.The criteria is as following:?1?all patients under 21 years old at diagnosis;?2?all patients diagnosed as thyroid papillary carcinoma postoperatively;?3?the original data and follow-up data of the cases complete.The clinicopathological characteristics and prognostic factors were analysed,and survival analysis was described in comparision.Results:94 PTC patients were stratified into prepubertal?n=15?and pubertal?n=79?groups.The prepubertal group tended to involve bilateral gland,be mulifocality,and progress with extrathyroid extension.Risk factors for prognosis were lateral cervical lymph node metastasis?p<0.001?,lung metastasis?p<0.001?,and therapeutic I131?p<0.001?.Survivalswithout reintervention at 3 and 5 years were,respectively,89.2%and81.0%.Conclusion:1.The prepubertal group tended to involve bilateral gland,be mulifocality,and progress with extrathyroid extension.2.Risk factors for prognosis were lateral cervical lymph node metastasis,lung metastasis,and therapeutic I131.3.recurrence occurred frequently within3 years,and children in the prepubertal group were more likely to recur.4.Children and adolescents,with lateral cervical metastasis and lung metastasis but without I 131 therapy,should be intensively treated and closely monitored.
