The Clinical Study On Repairing Soft Tissue Defects Of Extremities By Perforator Pedicled Propeller Flaps

Objective : To investigate the clinical effect of perforator pedicled propeller flap to repair soft tissue defect of extremities.Methods: A total of 24 patients with skin and soft tissue defects of extremities were selected as research subjects from November 2014 to September 2017,among them 8 are soft tissue defect of upper limb,and 16 are Soft tissue defect of lower extremities.All the patients reconstructed through the perforator pedicled propeller flaps,observing the flap clinical effect.All patients received the color Doppler ultrasound before operation,measuring and marking the diameter and length of the vessel as an ideal perforator vessel.The perforator vessel pedicled propeller flap was designed to repair the wound with the perforating vessel which was detected by Doppler and combined with the size of the wound.to observe the intraoperative and postoperative conditions Results: Among them 20 cases of direct suture of donor area after operation and 4 cases of free skin grafting,The distal of two flap occurred epidermis after operation,which were cured after dressing and.discontinuous line breaking.The othor flaps survived complelety.All patients were followed up for an average of 3-12 months.and all patients had recoveryed related functions..Conclusion: Color Doppler ultrasound can precisely locate the surface position of the perforating branch of peroneal artery in preoperative,decrease the operation risk,,guarantee the safety of the flap.The use of perforator pedicled propeller flap to repair wounds is relatively simple,with abundant blood supply,no need for vascular anastomosis,safe and effective,and high survival rate.It is a ideal method to repair skin and soft tissue defects of extremities.
