Objective:To analyze the clinical value of DNA ploidy analysis combined with liquid-based cytological test in screening cervical cancer and precancerous lesions.Methods:From October 2016 to January 2018,a total of 986 sexually active women who volunteered to go to the clinic or inpatients of the First Affiliated Hospital of Xinjiang Medical University took a new cervical screening clinic,DNA ploidy analysis of their cells revealed two reports,cell DNA ploidy analysis and cervical liquid-based cytological test(TCT).According to the inclusion and exclusion criteria of this study,895 women were selected as study subjects.Results Abnormal patients were recommended for cervical biopsy and follow-up regularly.Of these,188 cases had histopathological results,31 cases were uterine specimens,27 cases were cervical circumcision(LEEP knife)or cervical conization specimens,and 30 cases were cervical biopsy specimens.Histopathological diagnosis of CIN I was 5(25%),and CIN II was 8 cases(40%).CIN III was found in 5 cases(20%)and cervical squamous cell carcinoma(SCC)in 2 cases(10%).Cell DNA ploidy and TCT results were negative,but one case of cervical squamous cell carcinoma(SCC)was found by histopathological examination(biopsy sample).One case of CIN III(biopsy)and five cases of CIN II(total uterine specimen:1case)There were 1 circular or conic section,3 biopsies),2 cases of CIN I(1 case of cervical circumcision or conization,1 case of biopsy).To study the detection rate,sensitivity and specificity of DNA ploidy analysis combined with liquid-based cytology in cervical cancer screening for cervical cancer and precancerous lesions,using pathological examination as the gold standard and histopathological examination as the gold standard,and to study the detection rate,sensitivity and specificity of DNA ploidy analysis in combination with liquid-based cytology in cervical cancer screening,To analyse and evaluate the clinical efficacy of the two methods in cervical cancer screening alone and in combination.Results:869 cases(97.1%)were negative for cervical liquid-based cytology(TCT);TCT was positive in 26 cases(2.9%),including 12 cases of ASCUS(1.34%),5 cases of ASC-H(0.56%),and case 2(0.22%)of LSIL.In 188 specimens of histopathology,TCT was positive in 20 cases(10.6%),including 1 2 heteroploid cells in 3 cases,and 3 aneuploid cells in 9 cases;in 20 cases of TCT positive,11 cases were positive for histopathology,CIN Three cases of I(2 cases of ASC-H,1 case of HSIL)and CIN Of the 3 cases(1 case of ASCUS,1 case of LSIL,1 case of HSIL),CIN Of the 4 cases(1 case of ASCUS,1 case of ASC-H,2 cases of HSIL),1 case of cervical squamous cell carcinoma(SCC)(TCT was HSIL).Cell DNA ploidy analysis was negative in 837 cases(93.5%);the results of DNA ploidy analysis were positive in58 cases(6.5%),of which 32 cases(3.6%)were aneuploid cells,and 26 cases(2.9%)of 3 aneuploid cells.The results of DNA ploidy analysis were positive in 30 cases(16.0%),of which 12 cases were positive for histopathology,and CIN I had 3 cases of CIN Of the 3 cases,CIN Of the 5 cases of cervical squamous cell carcinoma(SCC),1 case.The sensitivity of TCT was 55%,the specificity was 94.6%,the positive predictive value was 55%,and the negative predictive value was 94.6%.Cells in the tissue samples of positive DNA ploidy analysis,the sensitivity of DNA was 60%,the specificity was 89.3%,the positive predictive value was 40%,and the negative predictive value was 94.9%.For statistical analysis,the 2 value was 6.75,P<0.05,the difference was statistically significant,which showed that the detection rate of this kind of detection method was significantly different,and the detection rate of DNA ploidy analysis was higher than that of TCT.If the result of TCT for ASCUS and/or cell DNA ploidy analysis is 1-2heteroploid cells,the diagnostic effect of combined detection is consistent with that of single cell DNA ploidy analysis.If TCT is the standard for ASC-H and above,and the results of DNA ploidy analysis?3 xenoid cells are the standard for biopsy,compare the diagnostic effect of the two methods,Statistical analysis showed that the value of?~2 was1.125,P>0.05.There was no significant difference between the two methods.This result was not consistent with the related literature.Conclusions:In this study,some of the data support cell DNA ploidy test better than the TCT result and some of them do not.This may be related to the lack of pathological tissue and the failure to take the pathological tissue out of the biopsy.Further investigation of the cause is needed,and further clinical diagnostic tests of a large number of samples are needed to confirm result.
