Objective: to analyze the efficacy and safety of endoscopic intervention and surgical treatment in elderly patients with choledocholithiasis(over 80 years old).Methods: a total of 131 elderly patients with choledocholithiasis who had received endoscopic intervention and surgical treatment in the first affiliated hospital of xinjiang medical university from January 2010 to December 2016 were collected.Among them,69 patients were treated with endoscopic intervention and 62 were treated with surgery.The efficacy and safety of the two surgical methods were compared according to the relevant evaluation indexes.Results: the surgical operation group was superior to the endoscopic interventional treatment group in terms of the success rate of primary stone removal and the incidence rate of primary pancreatic hyperamylasemia(P<0.05).Endoscopic interventional treatment group the intraoperative blood loss,operative time,postoperative drainage days,abdominal pain relief time,anal exhaust time,fast water days,hospitalization days,late postoperative blood,fever,infection of incision,than surgical repair group,the difference was statistically significant(P< 0.05);There was no statistically significant difference in overall cure rate of choledocholithiasis between the two groups(P > 0.05).Conclusion: endoscopic interventional therapy has certain advantages over traditional open surgery in elderly patients with choledocholithiasis: 1)less trauma.2)less complications.3)it can help control trauma,promote rehabilitation,facilitate postoperative care,reduce economic expenses,and reduce the number of hospitalization days.It is very suitable for minimally invasive treatment of elderly patients with choledocholithiasis.It is worthy of clinical application.It provides scientific basis for elderly patients with choledocholithiasis to develop individualized and optimal treatment methods.
