Objective: To study the MR imaging features and clinical relevance of vertebrobasilar prolongation.Methods: To recorded and analyzed the brain imaging data of 114 patients with VBD.Image data acquisition are done on the GE Discover 750 3.0TMR scanner.Scan sequences include high-resolution magnetic resonance imaging HRMRI,MR plain scan:T1WI,T2 WI,FLAIR,MRA,diffusion-weighted imaging DWI and magnetic susceptibility weighted imaging SWI.The maximum value of basilar artery diameter was measured on HRMRI images,and the imaging data of basilar artery bifurcation height,offset degree and vertebrobasilar artery type were recorded.1.114 patients were enrolled in this study divided into FVH group,FVH1 group,FVH2 group and FVH3 group by observing the presence or absence of high signal and high signal in FLAIR basal artery.Compare the differences of imaging and clinical data between the four groups.To observe the group of patients with HRMRI images of the basilar artery whether there is a plaque,divided into plaque group and no plaque group,compared the differences of patients imaging and clinical data in two groups.2.Observe the infarction and infarct size on DWI and T2 WI images of 78 patients with VBD,divide them into negative cerebral infarction group,lacunar infarction group,small infarction group and large infarction group.Compare the images and clinical information of the patients in each group.To observe whether there are CMBs in the posterior circulation area of SWI images of 30 patients,divide them into CMBs positive group and negative group,and to compare the imaging and clinical data of the two groups of patients.Results.1 Gender and basilar artery offset,men prone to migration,P=0.042.FVH and basilar artery diameter,degree of deviation was positively correlated as followed P<0.001,R=0.404,P<0.001,R=0.460.Correlation with VBD typing,P<0.001.There are no correlation with basilar artery bifurcation height,hypertension,diabetes,hyperlipidemia,smoking,P > 0.05.There are correlation with basilar artery plaque formation and VBD patients with vertebrobasilar artery offset degree,P < 0.001.No correlation with basilar artery diameter,bifurcation height,VBD with hypertension,diabetes,hyperlipemia and smoking,P>0.05.2 VBD patients with cerebral posterior circulation infarction and basilar artery diameter,degree of deviation graded,tortuous extension of the total score was positively correlated,as followed P=0.009,R=0.293,P < 0.001,R=0.492,P <0.001,R=0.506.And has a correlation with vertebrobasilar type,P<0.001.There is no correlation with both bifurcation height,hypertension,diabetes,hyperlipidemia and smoking,P>0.05.3 CMBs associated with basilar artery diameter,skewness and vertebrobasilar artery follow-up in VBD patients with cerebral infarction after acute cerebral infarction had correlations with the type of CMBs,as follow P=0.001,P=0.01,P=0.012.There is no correlation with bifurcation height,with hypertension,diabetes,hyperlipidemia,smoking.P>0.05.Conclusion:It is instructive to analyze the features of MR imaging in VBD for the clinical progression.
