Objective Investigate the effect of short-term prognosis of patients by the choice of surgical methods and timing of hypertensive intracerebral hemorrhage.Methods Retrospectively analyzed the clinical data of patients with hypertensive intracerebral hemorrhage in the Department of Neurosurgery,Handan first hospital from July 2015 to June 2017,setting the inclusion criteria and exclusion criteria,152 cases included in the case.It is divided into three groups according to the time of onset and the time of operation:the ultra-early group(≤6 hour,n=46),the early group(6~24 hour,n=67)and the delayed group(≥24 hour,n=39).The site of the disease is all in the basal ganglia.There are two kinds of surgical methods be adopt,one is to remove intracranial hematoma under the craniotomy by microscope,and the other is minimally invasive aspiration surgery.The GOS score of 1 months after operation was compared with three different operation time and two different surgical procedures(4~5 points were good for the short term effect).Results The results of GOS score of 1 months postoperatively showed that:The excellent rate of the super early group was 86.96%(40/46),slightly higher than early group 86.57%(58/67),and obviously higher than late group 48.72%(19/39),there is a significant statistical significance in the difference(P value < 0.01),but there was no statistical difference between the super early group and the early group(P value > 0.05).The GOS score of 1 months after operation shows the excellent rate between craniotomy group and minimally invasive surgery group(respectively 76.62%(59/77)、77.33%(58/75)),there were no statistically significant between them(P value > 0.05).Conclusion In order to obtain the preferable operative effect,we should choose appropriate surgical methods for individualized treatment for patients with hypertensive intracerebral hemorrhage who was in super early and early stage. |
