OBJECTIVE: To discuss the autophagy state of the HBV-ACLF and its HBV virus replication and the relationship with the severity of the liver diseases by texting the expression of the Beclin1 and LC3-?in the HBV-ACLF and the other chronic liver diseases related with hepatitis B virus.Applying for a new idea to treat and prevent for HBV-ACLF.METHODS: Collect the 20 cases of HBV-ACLF patients who were diagnosed and treated in our hospital during 2016 September and 2017 September,and the control groups are the same period of every 20 cases of CHB,LC,HCC and the healthy group.Collect the serum of all the research objects,and detect the expression of the autophagy related proteins of Beclin1 and LC3-? by ELISA,and collect the clinical data,such as virology,biochemical indicators and so on.Analyze the results by biology application software and Statistical Package for Social Sciences and compare the distinction between the autophagy related proteins of Beclin1 and LC3-?in different hepatitis B related chronic liver disease patients and relationship between the autophagy related proteins of Beclin1/LC3-?and HBV-DNA and the severity of liver diseases.RESULTS:(1)Autophagy related proteins of Beclin1 was all expressed in hepatitis B related chronic liver disease patients and the healthy group.The expression quantity respectively was(11.26ng/ml±3.51ng/ml)in HBV-ACLF?(8.73ng/ml±2.58ng/ml)in CHB?(11.21ng/ml±2.9ng/ml)in LC?(10.19ng/ml±3.33ng/ml)in HCC and(8.16ng/ml±2.51ng/ml)in the healthy group.Two groups were comparative analysis that the HBV-ACLF and CHB(P=0.009),HBV-ACLF and the healthy group(P=0.001),CHB and HCC(P=0.01)?LC and the healthy group(P=0.034),HCC and the healthy group(P=0.002)had statistically significant difference,the others were not statistically significant difference.(2)Autophagy related proteins of LC3-?was all expressed in hepatitis B related chronic liver disease patients and the healthy group.The data respectively was(67.11 ng/ml±19.46ng/ml)in HBV-ACLF,(60.71ng/ml±19.46ng/ml)in CHB?(55.01 ng/ml±19.0ng/ml)in LC?(49.74ng/ml±15.4ng/ml)in HCC and(46.53 ng/ml±17.67ng/ml)in the healthy group.Two groups were comparative analysis that the HBV-ACLF and LC(P=0.032),HBV-ACLF and HCC(P=0.02),HBV-ACLF and the healthy group(P=0.00),CHB and HCC(P=0.05),CHB and the healthy group(P=0.012)had statistically significant difference,the others were not statistically significant difference.(3)The capacity of HBV-DNA in the serum of the hepatitis B related chronic liver disease patients was respectively(2.92±0.56copy/ml)in CHB,(3.95±1.18copy/ml)in LC,(3.89 ±1.24copy/ml)in HBV-ACLF and(4.67±1.36copy/ml)in HCC.Two groups were comparative analysis that every two groups,except the HBV-ACLF and the LC group,had statistically significant difference(P <0.05).The correlation analysis of Spearman showed that there was positive correlation between Beclin1 and LC3-?(r = 0.29,p =0.03).There was positive correlation between Beclin1 and HBV-DNA(r = 0.42,p = 0.42),LC3-? was also positive with HBV-DNA(r = 1.95,P = 1.95).(4)The expression of autophagy related proteins of Beclin1 in the serum of the A,B,C group of Child-pugh was respectively(8.40ng/ml±2.71ng/ml),(10.62ng/ml±2.33ng/ml),(13.46ng/ml±3.12ng/ml).In the classification of the Child-Turcotte-Pugh score and the expression of Beclin1,the two-sample t test of the A group,B group and C group were statistically significant difference(p <0.05).The correlation analysis of Spearman between the autophagy related proteins of Beclin1 and MELD score showed that there was positive correlation between the autophagy related proteins of Beclin1 and MELD score(r = 0.494,p = 0.001).(5)The expression of the autophagy related proteins of LC3-? in the serum of the A,B,C group of Child-pugh was respectively(40.98ng/ml±11.30ng/ml),(62.30ng/ml±18.10ng/ml),(70.04ng/ml±16.04ng/ml).In the classification of the Child-Turcotte-Pugh score and the expression of LC3-?,the two-sample t test of the A group,B group and C group were also statistically significant difference(p < 0.05).The correlation analysis of Spearman showed that there was positive correlation between the autophagy related proteins of LC3-? and MELD score(r=0.529,p=0.00).CONCLUSIONS:(1)With the development of the hepatitis B related chronic liver disease patients,the expression of the autophagy proteins of Beclin1 and LC3-?increase first and then gradually decrease in the overall trend from the CHB to the HBV-ACLF.The number of the autophagy proteins Beclin1 and LC3-?expresses the most of all the groups in HBV-ACLF,which reveal that the autophagy is in the activated state.(2)In liver disease of CHB ? LC ? HBV-ACLF and HCC,the number of replication of hepatitis B virus becomes more and more while the disease develops.In the HBV-ACLF,the Beclin1 and LC3-? has a closely related relationship.At the same time,the higher the HBV-DNA,autophagy protein expression level is higher.(3)While the function of the liver become worse in the LC and HBV-ACLF,the expression of Beclin1 and LC3-?all increase,so that Beclin1 and LC3-?could be considered the close relationship with the severity of liver disease.
