Objective: To investigate the risk factors and thrombosis distribution characteristics of primary nephrotic syndrome with thromboembolism,and to summarize experience in order to prompt clinical work in the future.Methods: Collected the clinical materials of the patients meet the diagnosis Primary nephrotic syndrome in the First and Second Affiliated Hospitals of Guangxi Medical University in the period of 2015 to 2016.Patients were divided into embolism group and non-embolism group according to whether complicated with thromboembolism and analysis of risk factors and thrombosis distribution characteristics of PNS with thromboembolism.The elderly PNS patients were divided into embolization group and non-embolism according to whether there were no thrombosis,and analysis of risk factors for thromboembolism in elderly PNS patients.Results:1.In this study,a total of 736 patients with primary nephrotic syndrome were collected and 32 of them had thromboembolism.The incidence of TE in this study was 4.6%.2.Logistic regression analysis showed that 24-hour urinary protein,Fibrinogen,membranous nephropathy,history of hypertension and the use of glucocorticoid were the independent risk factors for patients with primary nephrotic syndrome complicated with thromboembolism.3.The thromboembolism sites were counted,including 17 cases(50%)of cranium,10 cases of limb(29.4%),4 cases of lung(11.8%),4 cases of kidney(11.8%),and 1 case of mesenteric and inferior vena cava.(2.9%).4.Among the 157 elderly patients with primary nephrotic syndrome,there were 12 cases of TE.The incidence of TE in elderly patients with PNS was 7.6% in this study.5.Logistic regression analysis showed that the use of glucocorticoid were the independent risk factors for elderly patients with PNS complicated with thromboembolism.Conclusion:1.24-hour urinary protein,fibrinogen,membranous nephropathy,history of glucocorticoid use and history of hypertension were independent risk factors for thromboembolism in patients with primary nephrotic syndrome.2.The history of glucocorticoid use was independent risk factors for thromboembolism in elderly patients with PNS.3.Thromboembolism in patients with primary nephrotic syndrome is common in the brain,followed by limb,lung,and kidney.
