Background: Temporal lobe epilepsy(TLE)is the most common form of epilepsy in the clinic. It is a chronic brain disease caused by multiple causes.Studies have confirmed the existence of executive dysfunction in patients with TLE,and the course of the disease on the brain function of TLE patients have a certain influence, but different course of the impact on the implementation of functional brain network changes have not been reported. In this study, the degree of centrality(DC)method was used to study the changes of the level of functional connections in patients with temporal lobe epilepsy(TLE)in different stages of the disease and their executive function. Methods: Using the attention network test(ANT),the executive function of 21 patients with TLE of ? 5 years,19 patients with THR of >5 years and 30 healthy controls was studied. The DC values of resting-state functional magnetic resonance imaging(rs-fMRI)were calculated for all subjects, and the correlation between the DC values of the extracted brain regions and the performance of control reactions was analyzed. Results: The efficiency of the control group in the patient group wassignificantly lower than that in the control group(P<0.05).Patients with a course of >5 years were more likely to have elevated DCs in the left insula than those with a course of ?5 years.Compared with the control group, patients with a course of ? 5 years had a decreased DC in the right hippocampus, and patients with a course of >5 years had a reduced DC in the right caudate nucleus and left frontal lobe. The elevated brain area of DC was located in the left central anterior gyrus and the central posterior gyrus.(P<0.01,Alphasim correction).Among them, the DC value of the right caudate nucleus in the patient group with >5 years disease duration was negatively correlated with the control network efficiency(P<0.05).Concussion: The executive function of TLE patients was impaired;the connection strength(node)of brain regions in patients with different disease durations was different. The executive function of patients with different disease durations was related to the connection of different brain regions. It was only found that there were compensatory effects in some brain regions of patients with longer course of disease. And the functional connection strength of the caudate nucleus is related to the execution function.
